动力髋部螺纹钉、Gamma钉治疗老年股骨粗隆间骨折的疗效分析

摘　　要：目的探讨动力髋部螺纹钉、Gamma钉两种内固定方法治疗老年性股骨粗隆间骨折的差异及疗效。方法回顾性分析2003年3月至2007年7月期间收治的92例高龄新鲜股骨粗隆间骨折患者的临床资料,分为动力髋部螺纹钉组和Gamma钉组。分别对手术切口、手术时间、术中出血量、术后颈干角丢失度等进行对比研究,同时对患髋进行综合评分来判断其疗效。结果术后随访4～52个月,平均23个月。Gamma钉组手术切口小,手术时间及术中出血量均明显少于DHS组(P<0.05)。结论动力髋部螺纹钉和Gamma钉是治疗老年股骨粗隆间骨折固定的理想材料,在控制手术创伤方面Gamma钉固定更优于DHS。Objective To compare and investigate the clinical effects of two internal fixation treating methods on the treatment of intertrochanteric fracture of femur in elderly patients.Methods 92 cases of fresh intertrochanteric fracture of femur admitted from March 2003 to July 2007 were reviewed retrospectively.All cases were divided into two groups：group A was treated with dynamic hip screw（DHS）,group B was treated with Gamma nail.Perioperaive indices of patients were statistically analyzed,such as operative wound,operation time,blood loss,femoral neck angle loss and so on.The medium-term or long-term curative effect of each treatment was assessed.Results After operation,the patients were followed up from 4 months to 52 months with 23 months on the average.There were significantly less injury,short operation time and less blood loss in group Gamma nail than that in DHS group（P〈0.05）.Conclusion DHS and Gamma nail are the ideal materials to fixation of senile intertrochanteric fracture of femur.Gamma nail is superior to DHS in terms of controlling operativetrauma.
