经尿道选择性绿激光前列腺汽化术与前列腺电切术治疗高龄良性前列腺增生症的临床疗效比较

摘　　要：目的比较经尿道选择性绿激光前列腺汽化术与前列腺电切术治疗高龄良性前列腺增生症的临床疗效。方法将88例良性前列腺增生症患者随机分为经尿道选择性绿激光前列腺汽化术(PVP)组和前列腺电切术(TURP)组各44例。分别行PVP术与TURP术。观察两组患者手术时间、住院时间、保留导尿管时间、术中术后出血、IPSS评分、最大尿流率及并发症。结果两组比较,手术时间无显著差异(P>0.05);住院时间及保留导尿管时间有显著差异(P<0.05);术中出血,输血比较有显著性差异(P<0.05);两组术前术后IPSS评分、最大尿流率均无显著差异(P>0.05);术后尿道刺激症、继发性出血、电切综合征、尿失禁等并发症方面有显著差异(P<0.05)。结论PVP术治疗高龄良性前列腺增生症安全有效,术中出血少、恢复快、症状改善快、并发症少。Objective To compare the efficacy of greenlight photoselective vaporization of prostate（PVP） and transuretharal resection of prostate（TURP）. Methods A total of 88 cases of BPH were divided into 2 groups;of them 44 cases underwent PVP and 44,TURP. The operative time, the length of hospital stay after operation, catheterization time,blood loss, IPSS, Qmax and complications were observed. Results Comparison between PVP group and TURP group included the following ：Two groups were similar in the operating time （P〉0.05）.Remarkable differences were found in length of hospital stay after operation, catheterization time, and intraoperative blood loss （P〈0.05）. No significant differences were found in IPSS and Qmax （P〉0,05）. Conclusions The PVP is safe and efficacious for older patients with BPH,with advantages of less blood loss, better tolerance and few complications .
