摘  要:目的:检测不同性别2型糖尿病患者正常体重组和肥胖组的血清视黄醇结合蛋白4(RBP4)和肿瘤坏死因子α(TNF-α)水平,探讨两者与体脂,糖类、脂类代谢,胰岛素敏感性等的相关性.方法:采用HOMA-IR评价各组胰岛素敏感性,测定受试者的体重指数(BMI),腰臀比(WHR),检测空腹状态下血清RBP4、TNF-α、血糖、HbA1c、血脂和胰岛素水平.结果:超重或肥胖者,RBP4及TNF-α浓度均比体重正常者高;男性RBP4浓度均比女性高,TNF-α浓度无性别差异.RBP4与TNF-α显著正相关.Pearson相关分析:不论性别,血清RBP4和BMI、WHR、TG及HOMA-IR均有显著正相关(P<0.05).而TNF-均与腹内型肥胖、脂代谢有关。Objective: To assay serum retinol binding protein-4 (RBP4) and TNF-α levels in patients with type 2 diabetes mellitus in different gender and weight, and to explore the correlation between the two factors and anthropometric parameter, glucose and lipid metabolism parameters and insulin sensitivity index etc. Methods: insulin sensitivity was assessed by HOMA-IR. Meanwhile the anthropometrical parameters such as body mass index (BMI) and waist hip ratio (WHR) were evaluated and the serum RBP4, TNF-α, glucose, HbAlc, lipids and insulin levels were also measured in fasting status. Results: Serum RBP4 and TNF-α levels were significantly increased in patients with obesity; Serum RBP4 level in male was significantly higher than that in female while TNF-α level had no difference in different gender. There was also a positive correlation between RBP4 and TNF-α. Pearson correlation analysis showed that serum RBP4 correlated positively with BMI, WHR, TG and HOMA-IR (P 〈 0.05). TNF-α which had a negative correlation with HDL-C (P 〈 0.05), correlated significantly with all index but HbAlc and LDL-C. Conclusion: Both of RBP4 and TNF-α correlate with WHR and lipid metabolism, TNF-α also has a correlation with glucose metabolism. There maybe synergistic effect of RBP4 and TNF-α in insulin resistance.
