慢性阻塞性肺病患者右室Tei指数与肺功能指标的相关性分析

摘　　要：目的探讨慢性阻塞性肺病(COPD)患者右室Tei指数与肺功能指标是否存在相关关系。方法回顾性分析了14例临床症状轻重程度不同的COPD患者,将这些病例经超声心动图测得的右室Tei指数分别与其肺功能指标:第1秒用力呼气量与用力肺活量比值(FEV1/FVC)、一氧化碳弥散度与预计值比值(DLCO/P)、氧分压(PaO2)、二氧化碳分压(PaCO2)进行直线相关性分析。结果14例COPD患者的右室Tei指数与FEV1/FVC呈高度负相关,r=-0.851,p<0.001;与DLCO/P呈中度负相关,r=-0.638,p<0.05;与PaCO2呈中度正相关,r=0.703,p<0.01,与PaO2的相关关系不明显,r=-0.345,p>0.05。结论COPD患者的心功能受损与肺功能受损存在相关性。右室Tei指数是反映右心功能较敏感的指标。Objective To investigate the relationship between right ventricular Tei index and lung function indiees in patients with chronie obstructive pulmonary disease（COPD）.Methods A retrospective analysis of 14 cases of COPD with different degree on clinical symptoms was performed.The correlation between their right ventricular Tei index on echocardiography and lung function indiees （FEV/FVC,DLCO/P.PaO2.PaCO2）was performed.Results There was significant negative relations between right ventricular Tei index and FEV/FVC in 14 cases of COPD（r=-0.851.P〈0.001）.and between right ventricular Tei index and PLCO/P in 14 cases of COPD（r=-0.0638 P〈0.05）.There was signifieant positive relation between right ventricular Tei index and PaCO2 in 14 cases of COPD （r=-0.703,P〈0.01）.There was no significant relation between right ventricular Tei index and PaO2 in 14cases of COPD （r=-0.345.P〉0.05）.Conclusions There are correlation between heart function discorder and lung function discorder in patients with COPD .The right ventricular Tei index is a quite sensitive indieator for right ventricular function.
