摘  要:目的研究核因子κB(nuclear factor-κB,NF-κB)与内毒素(lipopolysaccharide,LPS)诱导的大鼠急性肾损伤(acute kid-neyinjury,AKI)发病的关系及初步探讨乌司他丁(ulinastatin,UTI)对大鼠AKI时肾脏的保护作用。方法健康雄性Wistar大鼠30只随机分为3组:健康对照组经尾经脉注射1ml生理盐水;LPS组经尾经脉注射LPS5mg/kg体重,复制AKI模型;LPS+UTI组在按与LPS组相同标准给LPS的同时给予UTI100kU/kg体重。观察6h,取一侧肾组织免疫组化法检测NF-κB的含量;测定血肌酐、尿肌酐,结合尿量计算内生肌酐清除率(creatinine clearance rate,Ccr);观察各组大鼠光镜及电镜下肾脏的病理改变。结果LPS组大鼠Ccr较正常对照组和LPS+UTI组显著降低,差异有统计学意义(P<0.01),LPS+UTI组与健康对照组相比无统计学意义(P>0.05)。LPS组大鼠肾组织中NF-κB的表达明显高于健康对照组(P<0.01),LPS+UTI组高于健康对照组(P<0.05),但低于LPS组(P<0.01)。光镜检查UTI可使肾小管上皮细胞浊肿等组织病理改变明显减轻,电镜下可见线粒体损伤亦显著减轻。结论UTI可降低肾组织中炎性细胞因子NF-κB的水平,对LPS诱导大鼠AKI时肾脏具有保护作用。Objective To investigate the relations between nuclear factor-κB(NF-κB)and the development of lipopolysaccharide(LPS)-induced acute kidney injury(AKI),and to explore the effect of ulinastatin(UTI)on AKI.Methods Thirty male Wistar rats were randomly divided into three groups:control group,LPS group and LPS+UTI group.The rats were injected with normal sodium 1 ml via caudal vein in control group,with LPS 5 mg/kg via caudal vein to establish AKI model in LPS group,with UTI 100 kU/kg after injection with LPS in LPS+UTI group.Immunohistochemistry was used to determine the concentration of NF-κB in kidney tissue in rats at 6 h after LPS-induced injury.The creatinine clearance rates(Ccr)were measured and the morphological changes of kidney were observed under light microscope and electron microscope.Results Compared with control group and LPS+UTI group,Ccr significantly decreased in LPS group(P〈0.01),but there was no significant difference between control group and LPS+UTI group(P〉0.05).The concentration of NF-κB in LPS group was significantly higher than in control group and LPS+UTI group(P〈0.01).The level of NF-κB in LPS+UTI group was little higher than in control group(P〈0.05).Pathological section and microscope section showed that the inflammation reaction of kidney in LPS+UTI group was lighter than in LPS group.Conclusion UTI may alleviate LPS-induced inflammatory reaction and damage to kidney in rats.
