摘　　要：目的探讨1-磷酸鞘氨醇(S1P)对大鼠急性坏死性胰腺炎肺损伤(ANP-LI模型组)的保护作用及机制。方法36只Wistar大鼠随机分为假手术组、模型组、S1P处理组(S1P组)。3组动物于术后6h留取标本作如下实验:(1)测定血清淀粉酶和血脂肪酶。(2)测定左肺组织髓过氧化物酶(MPO)。(3)行左肺组织病理学检查。(4)取左肺进行肺泡灌洗测定蛋白含量;细胞沉淀涂片行白细胞和中性粒细胞计数。(5)行胰腺组织病理学检查。结果S1P组在造模后6h,肺组织MPO水平、肺泡灌洗液蛋白含量、肺泡灌洗液白细胞及中性粒细胞计数均显著低于模型组(均为P<0.01);肺组织病理组织学改变显著减轻。与模型组比,S1P组血清淀粉酶、脂肪酶及胰腺病理无明显改变。结论S1P对大鼠ANP-IL有明显的保护作用;其有可能作为临床治疗的有效手段。Objective To investigate the protective effects of sphingosine- 1 - phosphate （ S 1 P ） on lung injury （L1） in acute necrotizing pancreatitis rats. Methods Thirty-six Wistar rats were allocated to sham operation group, acute necrotizing pancreatitis model group and S1 P pretreatment group. Bronchoalveolar lavage fluid （ BALF ） and tissues from the left lung and pancreas were collected 6 h after the model induction. BALF protein content, BALF leucocyte and neutrophil count, and lung tissue myeloperoxidase （MPO） activity were examined. Serum amylase and lipase were determined. In addition, histopathological changes of the pancreas and lung were observed under light microscope. Results BALF protein content, BALF leucocyte and neutrophil count, and lung tissue myeloperoxidase （ MPO ） activity were all reduced significantly in S 1 P pretreatment group compared to those in acute necrotizing pancreatitis model group （ P 〈 0. 01 ）. The pathological injury of the lung was obviously less severe in S 1 P group compared to that in model group. The serum amylase and lipase levels and the pathological damage of the pancreas were not reduced in S 1 P group. Conclusions S1 P has a significant protective effect on LI in SAP rats, and it may be used as a clinical therapeutic strategy for acute necrotizing pancreatitis.
