髋关节蛙形位透视在股骨颈骨折内固定手术中的应用及相关问题探讨

摘  要:目的探讨股骨颈骨折闭合复位内固定手术中的透视体位及相关问题。方法对2004—2006年间收治的33例股骨颈骨折病人进行回顾性分析。结果30例患者行患髋蛙形位透视下闭合复位空心螺钉内固定手术;3例行人工关节置换术。30例病人获平均16.5个月(8—36个月)随访,X线均示骨折解剖复位,螺钉空间位置良好,后期随访28例骨性愈合,1例骨折不愈合,1例发生股骨头缺血性坏死。结论患髋蛙形位透视下辅助复位可最大程度的复位;新鲜股骨颈骨折的首选治疗方法是骨折复位空心螺钉内固定术。Objective: To explore the body postion and associated problem during the treatmeat of femoral neck fractures by closed reduction and intemal fixation. Methods : A retrospective review was done in 33 cases of femoral neck fractures from 2004 to 2006. Results : Closed reduction and carmulated screw fncation assisted by fluoroscopy of frog position was done in 30, total hip arthroplasty (THA) in 3. In the average 16.5 months ( ranging between 8 to 36 months) follow - up time, all the postoperative plain films showed anatomical reduction, good space location of screws, bony union in 28, bony non - union in 1 and ostecnecrosis of femoral head in I. Conclusion: Reduction assisted by fluoroscopy of hip - frog - shaped position can obtain the perfect reduction. The first choice in the treatmeat of the fresh femoral neck fracture should be reduction and carmulated screw fixation.
