摘  要:目的探讨冠心病患者脉象客观特征,旨在客观评估脉象信息在冠心病诊断和疗效评价中的意义。方法运用脉象检测系统采集健康人、冠心病、非冠心病患者的脉图,采用Hilbert-Huang变换(HHT)方法和时域方法分析患者的脉图信息。结果冠心病患者脉图存在特征性变化:①脉图时域参数h1、h3、h4、h3/h1、t、t5/t4升高,w1增宽。②HHT方法分析显示冠心病患者C2波形杂乱且波幅高低不齐,占44%;C3波形欠规则且平均幅度超过10克力,占72%。与健康组比较差异有统计学意义。结论时域方法和HHT方法提取的脉图波形特征可作为冠心病诊断和辨证的客观依据;预示脉诊这种简便易行且无创的检测方法,能够对冠心病的发生进行辅助监测。Objective To study objective characters of pulse tracings in CHD,for objectively evaluate sense of pulse signal to diagnosis and appreciation of therapeutic effect in patients of coronary heart disease (CHD), and accordingly provide theoretic proof for developing non-invasive technique of pulse diagnosis. Methods Using the pulse detection system,pulse graph in CHD patient, patients without CHD and "health" adult were collected and compared.Then characters of the pulse signal were analyzed with Hilbert-Huang transformation routine (HHT) and time domain method,respectively. Results There existed characteristic change in pulse graph in CHD. (1)h1,h3, h4,h3/hl ,t,t5/t4 in time domain parameter of pulse graph were heighten,and wl widened.(2)There have 44% of C2 wave in HHT display confused and disorderly wave and irregularly wave amplitude in CHD.There have 72% of C5 wave in HHT exhibit irregular wave and wave amplitude over 10 gram-force.These changes were significantly different with health adults. Conclusion Characteristic wave of pulse graph extracted with methods of time domain or HHT routine might consider as objective proofs for diagnosis and differentiation in CHD. Our researches prognosticate that pulse diagnosis can be used as an ancillary determination in occurrence of CHD for reasons of the advantage of convenient operation and non-invasion.
