摘 要:目的:观察糖尿病(DM)大鼠肾小球足细胞的表达及尿液足细胞(UPC)排泄的改变,及罗格列酮对DM大鼠肾小球足细胞分布及UPC排泄的影响。方法:雄性Wistar大鼠随机分为正常对照组和糖尿病模型组;后者经STZ诱导糖尿病模型成功后再随机分为糖尿病组和治疗组(罗格列酮5mg·kg-1·d-1)。实验10w末测24h尿蛋白定量(TP)、血糖(BG)、肾素活性(RA)、血管紧张素Ⅱ(AngⅡ)和醛固酮(Ald)水平。间接免疫荧光法检测尿沉渣足细胞特异性标志蛋白PCX(podocalyxin)以观察UPC排泄水平,检测肾小球上皮细胞蛋白-1(GLEPP1)以观察肾小球足细胞分布。结果:DM组及治疗组UPC、TP、BG、PRA、AngⅡ、Ald水平较对照组明显升高(P<0.01)。与DM组相比,治疗组TP、BG、AngⅡ显著降低(P<0.01),UPC亦降低(P<0.05)。病理组织肾小球荧光染色示GLEPP1在对照组正常,DM组明显缺失,治疗组呈节段性缺失。UPC与TP、AngⅡ呈正相关(r=0.41,0.38,P<0.05),而与BG、PRA、Ald无显著相关性(r=0.19,0.23,0.13,P>0.05)。结论:肾小球足细胞缺失参与糖尿病肾病(DN)的发病。罗格列酮可减轻DN大鼠尿蛋白,减少足细胞脱落及尿液足细胞的排泄。Objective: To observe the expression of podocyte in glomerulus rulus and the excretion of urinary podocytes(UPC), and the influence of rosiglitazone on the distribution of podocytes in glomerulus and UPC excretion in rats with diabetic mellitus (DM). Methods:Wistar rats were randomly divided into two groups: normal control group and DM model group. After the DM models were successfully established by intmperitoneal injection of streptstreptozotocin in mrs, the animals were randomly divided into two groups: DM group without therapy and DM group with rosiglitazone(5mg · kg^-1 · d^-1 ). The levels of 24 h total proteinuria(TP), blood glucose(BG), renin aetivity(RA), angiotensinⅡ(Ang) and aldosterone(Ald) of all animals were measured at the end of the 10th weeks. Podocyte - specific marker protein - podocalyxin(PCX) was detected in urinary .sediment by immunofluorecene to observe the excretion of UPC; glomerular epithelial cell membrane protein - tyrosine phosphatase 1 (GLEPP1) was detected to observe the distribution of podocytes in glomerulus Results:The levels of UPC, TP, BG, RA, Ang, Aid in DIM group and DIM group treated with rosiglitazone were significantly higher than those in normal control group(P〈0. 01). The level of TP, BG, AngⅡand UPC in DM group treated with rosiglitazone was lower than those in DM group(P〈0. 01 -0. 05). Histologically, the expression of GLEPP1 was normal in oontrol group in glomeruli by immunofluorescene, but it was obviously absent in DM group, was partly absent in the group treated with rosiglitazone; The levels of UPC showed a positive correlation with TP and AngⅡ(rs =0. 41, 0. 38, P〈0. 05), has not correlation with BG, PRA and Ald(rs =0. 19,0. 23,0. 13,P〉0. 05). Conclusion:Podocyte loss in glomerulus was involved in the efiopathogenisis of diabetic nephropathy(DN). Rosiglitazone oould contribute to decrease proteinuria and reduce the shedding and exerection of podpcyte.
