血清C-反应蛋白与冠心病相关性的研究

摘　　要：目的:探讨超敏血清C-反应蛋白(high-sensitivity c-reactive protein,hs-CRP)与冠心病(coronary heart disease,CHD)的相关性。方法:随机选取138例经冠状动脉造影证实的冠心病患者,其中74例稳定型心绞痛(SA)、41例不稳定型心绞痛(UA)、23例急性心肌梗死(AMI),25例健康者作为对照。用乳胶增强免疫透射比浊法(终点法)检测血清其hs-CRP含量。同时探讨hs-CRP与肌钙蛋白I(cTnl)、肌酸激酶(CK)、肌酸激酶同工酶(CKMB)的关系。并参考比较不同hs-CRP水平各组患病数。结果:冠心病各亚组hs-CRP水平均高于对照组(P<0.05);UA组血清hs-CRP浓度显著高于SA组;AMI组血清hs-CRP浓度分别显著高于UA组与SA组,均有统计性差异(P<0.05)。结论:冠心病患者血清hs-CRP水平升高,是冠心病的一项独立危险因子,与冠状动脉粥样硬化斑块内的炎症反应有关;血清hs-CRP水平对冠心病的病变程度具有预测价值。Objective：To investigate the clinical value of high - sensitivity C - reactive protein（hs - CRP） in the diagnosis of coronary heart diseases（CHD）. Methods： 138 cases of CHD （including 74 cases of stable angina（SA）, 41 ones of unstable angina pectoris（UA） and 23 ones of acute myocardial infarction（AMI） and 25 healthy subjects were enrolled in this study. The contents of their serum hs - CRP, troponin Ⅰ（cTnⅠ）, creatine kinase（CK） and MB isoenzyme of creatine kinase（CKMB） were examined. The relationship between hs CRP and cTnⅠ, CK, CKMB were studied. The changes of hs - CRP in the patients with different type of CHD were investigated. Results：The level of serum hs - CRP of CHD group was significantly higher than that of the healthy group（P〈0. 05）. Among CHD group, the level of serum hs - CRP of patients with AMI was the highest and UA was higher than that of SA with a significant difference （P〈0. 05）. Conclusion： Elevation of serum hs - CRP is an independent risk factor of CHD; the inflammation in the atheroselerotic plaque is involved in development of CHD; serum hs - CRP can be used as a prediction index of CHD.
