摘 要:目的探讨类风湿因子(RF)对一组致畸病原体免疫球蛋白M(TORCH-IgM)抗体检测结果的影响。方法对89份RF阳性血清先用捕获法进行TORCH-IgM抗体检测,然后将所得阳性标本用变性IgG中和,再稀释标本。将这两种方法处理后的标本再进行TORCH-IgM抗体检测。结果(1)89份RF阳性血清检出弓形虫IgM抗体(抗-TOX-IgM)8份(9.0%),巨细胞病毒IgM抗体(抗-CMV-IgM)3份(3.4%),风疹病毒IgM抗体(抗-RUV-IgM)1份(1.1%),单纯疱疹病毒IgM抗体(抗-HSV-IgM)2份(2.2%)。(2)阳性标本被不同剂量变性IgG中和后,除1例TOX-IgM和CMV-IgM仍为阳性不被中和,标本被稀释到1∶1 600 TOX-IgM和CMV-IgM仍为阳性外,其余标本在被中和及稀释后TORCH-IgM检测均转为阴性。变性IgG用量和标本的稀释度与RF值有相关性(r=0.921、0.921,P<0.05)。结论RF对TOX、CMV、RUV、HSV IgM抗体检测存在不同程度的干扰,对TOX-IgM的干扰最明显;TORCH-IgM抗体检测阳性标本用变性IgG中和时应考虑RF值。Objective To investigate the effect of rheumatoid factor(RF) on the detecting results of TORCH- Ig M. Methods TORCH-Ig M was detected in 89 serum samples of positive RF by capture method, then the positive samples were neutralized with denatured Ig-M and diluted. TORCH-Ig M was detected in the samples treated with these two methods. Results (1)8 cases of TOX-Ig M ( 9.0 ~//00 ), 3 cases of CMV-Ig M (3.4 %), 1 case of RUV-Ig M (1.1%) and 2 caes of HSV-Ig M(2.2%) were detected among 89 samples of positive RF. (2)After the positive samples were neutralized with different doses of denatured IgM, the detection of Ig-M all changed to negative after neutralization and dilution, except 1 case of positive TOX-Ig M and CMV-Ig M was not neutralized and remained positive when the sample was diluted to 1 : 1 600. The amount of denatured Ig-M and dilution of the sample were related to the RF value(r=0. 921,0. 921,P〈0.05) Conclusion The interference of RF on the detection of TOX, CMV, RUV, HSV IgM exists in different extents, especially on TOX-Ig M. RF value should be considered when the positive serum sample of TORCH-Ig M antibody is neutralized with denatured Ig G.
