康复期男性海洛因依赖者注意力与听觉P_(300)的研究

摘　　要：目的探讨处于康复期的海洛因依赖者注意力与听觉P300的特点以及二者的关系。方法以数字划销测验(NCT)评估38例康复期的男性海洛因依赖者(MPHD)和14例健康对照的注意力,同时以脑诱发电位仪检测康复期MPHD及健康对照前额区(Fz)、中央区(Cz)及顶区(Pz)听觉P300。结果康复期MPHDNCTⅠ阶段、Ⅱ阶段、Ⅲ阶段、Ⅴ阶段净分及总净分均显著低于对照组(P<0.05或P<0.01);Cz点AEP-P300波幅显著低于健康对照组(P<0.05)。MPHDFz、Cz点AEP-P300波幅与净分Ⅴ呈正相关(P<0.01),在Pz点AEP-P300波幅与净分Ⅲ呈正相关(P<0.05);在Cz点AEP-P300潜伏期与净分Ⅰ及净分Ⅲ呈正相关(P<0.05)。结论康复期MPHD存在明显的注意力损害与皮质功能的紊乱,AEP-P300可在一定程度上反映海洛因依赖者注意力损害的程度。Objective To explore the feature of attention and auditory P300 for male patients with heroin dependence（MPHD） in a stage of rehabilitation. Methods A total of 38 MPHD and 14 healthy controls were assessed with Number Cancellation Test （NCT）, and with Neuroscan event related potential apparatus at forehead zone （Fz）, central zone （Cz） and parietal zone （Pz） for auditory evoked potential P300 （AEP-P300）. Results Net scores of the Ⅰ , Ⅱ , Ⅲ, Ⅴ stage and total net score of NCT in MPHD were significantly lower than those in healthy controls （P〈0.05 or P〈0.01）. MPHDshowed significantly lower AEP-P300amplitude at Cz than healthy controls（P〈0.05）. At Fz and Cz, AEP-P300amplitude showed significant positive relation with net score V of NCT （P〈 0. 01）, and at Pz, AEP-P300 amplitude showed significant positive relation with net score, Ⅲ of NCT in MPHD（P〈0.05）. AEP-P3o0 latency showed significant positive relation with net scoreⅠ and Ⅲ of NCT in MPHD（P〈0.05）. Conclusion MPHD suffer from decreased attention and cortex dysfunction. AEP-P300might reflect the degree of attention impaired in some extent.
