摘  要:目的探讨祛瘀生肌法对糖尿病溃疡大鼠创面愈合中凋亡相关基因Bax、Bcl-2mRNA表达的影响。方法48只SD大鼠采用四氧嘧啶造成糖尿病模型后,剪去背部局部全层皮肤造成糖尿病溃疡。将大鼠随机分成生肌化瘀方1组、生肌化瘀方2组、生肌化瘀方3组和生理盐水对照组。分别于创面修复第3天、11天取材,采用实时PCR法检测新生肉芽组织中凋亡相关基因Bax和Bcl-2mRNA变化。结果创面修复第3天,生肌化瘀方1组和生肌化瘀方2组Bcl-2拷贝数及Bax/Bcl-2比值均高于生理盐水对照组(P<0.05);创面修复后第11天,生肌化瘀方1组Bcl-2拷贝数和Bax/Bcl-2比值仍高于生理盐水对照组(P<0.05),而生肌化瘀方2组Bax/Bcl-2比值与生理盐水对照组差异无统计学意义(P>0.05)。结论祛瘀生肌中药调节糖尿病溃疡大鼠模型新生肉芽组织中凋亡相关基因Bax和Bcl-2mRNA表达是促进创面愈合的原因之一。早期重用化瘀药物能抑制细胞凋亡,有利于创面愈合,而晚期重用生肌药物能促进细胞凋亡,有利于减少疤痕形成。Objective To study the effect of Stasis-removing and muscle-regeneration therapy on the expression of Bax and Bcl-2 mRNA in granulation tissue of diabetic rats wound healing. Methods Totally 48 male SD diabetic rats with full-thickness skin lesion were randomized into 4 groups: Shengji Huayu (Muscle-regeneration and stasis-removing) Recipe-treated group 1,2,3 and normal saline control group. The tissues for test were collected on the 3rd and llth day of the wound healing and the apoptosis-related gene Bax and Bcl-2 mRNA in granulation tissue were tested with real-time PCR method. Results On the 3rd day of wound healing,Bcl-2 mRNA copies and Bax/Bcl-2 mRNA of the rats in Shengji Huayu Recipe-treated group 1 and 2 were higher than those in the control group (P^0. 05). On the llth day of wound healing, Bcl-2 mRNA copies of the rats in Shengji Huayu Recipe-treated group 1 was still higher than that in the control group (P〈0.05), but Bax/Bcl-2 mRNA of Shengji Huayu Recipe-treated group 2 was not significantly different from that of control group (P〉0. 05). Conclusion Stasis-removing and muscle-regeneration herbs may promote wound healing in diabetic rats through regulating Bax and Bcl-2 mRNA expression in granulation tissue. At the early stage, using more stasis-removing herbs ean inhibit the apoptosis, benefiting the wound healing, while using more muscle-regeneration herbs can promote the apoptosis at the late stage, benefiting the reduction of scar formation.
