瑞芬太尼异丙酚对气管插管血管反应的影响

摘　　要：目的比较不同剂量瑞芬太尼复合异丙酚麻醉诱导对气管插管时的心血管反应的影响。方法ASAⅠ～Ⅱ级择期全麻手术患者60例按瑞芬太尼剂量不同随机分为3组:1μg/kg(Ⅰ组)、2μg/kg(Ⅱ组)、3μg/kg(Ⅲ组),各组麻醉诱导用药依次静注异丙酚2μg/kg,维库溴胺0.1μg/kg,瑞芬太尼1μg/kg(Ⅰ组)、2μg/kg(Ⅱ组)、3μg/kg(Ⅲ组)后,经口气管内插管。观察诱导前、诱导后、插管后1、3、5、10min的血压(SBP、DBP、MAP)和心率(HR)变化,对所得数据进行统计学分析。结果各组患者诱导后及插管后1、3、5、10min的血压(SBP、DBP、MAP)与诱导前相比有显著性降低(P<0.05或P<0.01);组间比较,第Ⅲ组比第Ⅰ、Ⅱ组有显著性差异(P<0.05)。各组心率(HR)无显著性差异(P>0.05)。结论1、2μg/kg瑞芬太尼复合异丙酚能有效抑制气管插管引起的心血管不良反应。Objective To compare the cardiovascular response after anesthesia induction with different dosage remifentanil in combination with propofol. Methods Sixty ASA Ⅰ～Ⅱ patients undergoing electived surgery under general anesthesia were enrolled in this study. The patients were divided randomly into three group ： 1 μg/kg （ group Ⅰ ）, 2 μg/kg （ group Ⅱ）, 3 μg/kg （ group m ）. The patients were administered propofol 2μg/kg and vecuronium 0.1 μg/kg and remifentanil 1 μg/kg （group Ⅰ ）, remifentanil 2μg/kg （group Ⅱ ） or remifentanil 3μg/kg（group m） as a rapid bolus injection. SBP,DBP, MAP, HR were analyzed statistically at all time. Results SBP DBP MAP decreased significantly in three groaps after induction of anesthesia and lasted 10minute after post -intuhation （ P 〈 0.05 orP 〈0. 01）, Groupm decreased significantly than group Ⅰ or group Ⅱin either group （ P 〈 0.05）. HR did not changed significantly in three groups or in either group. Conclusion Remifentanil 1,2μg/kg in combination of propofol can depress cardiovascular response effectively.
