输尿管镜下双频双脉冲Nd∶YAG激光治疗急性梗阻性肾功能衰竭——附39例报告

摘　　要：目的探讨输尿管镜配合双频双脉冲Nd∶YAG激光碎石术治疗上尿路梗阻合并急性肾功能衰竭的可行性。方法34例输尿管结石引起梗阻患者在输尿管镜直视下采用双频双脉冲激光碎石。2例卵巢癌术后放疗造成腹膜后纤维化引起输尿管梗阻,3例输尿管结核造成输尿管狭窄引起输尿管梗阻,均在输尿管镜下放置D-J管。结果39例均得到满意疗效。结论输尿管镜配合双频双脉冲激光碎石是目前治疗上尿路梗阻并发急性肾功能衰竭的一种安全、微创、有效的方法。Objective To study the probability of frequency-doubled-double-pulse Nd ： YAG laser （FREDDY） by ureteroscopy in the treatment of acute renal failure due to upper urinary tract obstruction. Methods The data of 34 cases with upper urinary tract obstruction due to ureteral stone were performed frequency-doubled-double- pulse Nd ： YAG laser lithoclasty by ureteroscopy. Two patients due to massive retroperitoneal fibrosis resulting from radiation therapy after ovarian carcinoma surgery, and three patients with ureterostenosis due to ureteral tuberculosis were performed ureteroscopical dilatations indwelled double-J stents. Results All thirty-nine patients had satisfactory clinical results. Conclusion Frequency-doubled-double-pulse Nd＇YAG laser lithoclasty by ureteroscopy is a safe, efficacious and minimally-invasive choice in the treatment of acute renal failure due to upper urinary tract obstruction.
