摘　　要：目的探讨ABO血型不相容造血干细胞移植后患者对输注血小板的血型选择。方法回顾性分析34例ABO血型不合异基因造血干细胞移植后血型转换格局。结果ABO血型主要不相容移植后,患者正、反定型将转换为供者血型,ABO次要不相容及主次均不相容时,患者转换后的ABO血型正反定型不一致。结论对ABO血型不合患者移植后输血,应权衡所选血型对造血重建的影响并根据即时的ABO血型抗体效价选择合适的血产品血型。Objective To investigate the blood type selection of platelet for transfusion in patients after ABO-incompatible hematopoietic stem cell transplantation. Methods Thirty-four patients who received heterogenic hematopoietic stem cell transplantation were reviewed retrospectively, and then the style of blood type switch was analyzed. Results After transplantation with stem cells from ABO major incompatible donors, the positive and negative blood type determination of patients switched to the blood type of the donors. On the other hand, after transplantation with stem cells from ABO minor incompatible or bidirectional incompatible donors, the positive and negative blood type determination of switched ABO blood type was incompatible in every patient. Conclusions For blood transfusion in patients who received ABO incompatible transplantation, the effect of selected blood type on hematopoietic restitution should be weighed. Then appropriate blood type of blood products should be selected based on the titer of ABO blood type antibody.
