摘  要:【目的】探讨神经节甘脂(GM-1)对早产儿脑损伤保护作用及临床疗效。【方法】82例发生脑室周围一脑室内出血(PVH-IVH)和脑室周围白质软化(PVL)脑损伤的早产儿分为GM-1组(42例)和对照组(40例)。GM-1组在常规治疗基础上加用GM-1治疗。观察与比较两组临床主要症状呼吸暂停发生率和吸吮-吞咽能力,并分别于校正胎龄40周和41周时予新生儿神经行为评分(NBNA)。【结果】GM-1组两次测查均明显高于对照组(P均<0.01)。临床主要症状吸吮-吞咽能力明显优于对照组(P<0.05),呼吸暂停发生率显著低于对照组(P<0.05)。【结论】GM-1为中枢神经系统损伤修复药物,对脑损伤的早产儿神经行为发育有促进作用,可改善近期预后,提高生存质量。[Objective] To study the clinical curtive and protective effects of GM-1 on treating the premature infants with brain damage. [ Methods] 82 neonates with PVH-IVH and periventricular leucomalacia(PVL) were divided into GM-1 treated group (n=42) and control group (n=40). On the basis of positive treatment,the first group meanwhile accepted the treatment of GM-1. A dynamically observation was made on the rates of apnea and a comparison with the babies' ability of suck and swallow between the two groups. Neonatal behavioral neurological assessments (NBNA) for all infants were performed separately in 40 weeks after correcting gestational age and in 41 weeks. [Results] The NBNA scores of GM-1 group both in 40 weeks and 41 weeks were significantly higher than those of control group (P〈0.01). The infants' ability of suck and swallow was better in GM-1 group than that in control group(P^0.01). The rate of apnea was lower in GM-1 group than that in control group (P〈0.01). [Conclusions] The GM-1 can rehabilitate the damaged neurological system of premature infants with brain damage. Early administration can promote development of the neurobehavioral, improve prognosis in the early life and the quality of their life.
