摘  要:目的探讨髓样细胞触发受体-1(TREM-1)基因表达与脓毒症所致多器官损害的关系。方法采用小鼠盲肠结扎穿孔(CLP)造脓毒症模型。将60只雄性小鼠随机分为正常对照组(n=10)、假手术组(n=10)和脓毒症组(n=40),并按不同的时间点留取肝、肺、肾、小肠组织及血液样本,分别用于检测TREM-1mRNA,TNF-αmRNA表达及测定丙氨酸转氨酶(ALT)、肌酐(Cr)、肺组织髓过氧化物酶(MPO)、小肠组织二胺氧化酶(DAO)的活性。结果与对照组及假手术组相比,CLP后6h肝、肺、肾组织TREM-1 mRNA表达开始增多,肝组织24 h表达均达峰值(P<0.01),肺、肾组织12 h表达达峰值(P<0.01),小肠组织的TREM-1表达表现出了一定的升高趋势,但差异无显著性(P>0.05)。TNF-αmRNA在各组织的表达在CLP后6h开始升高(P<0.05或P<0.01),12h降低,48h再次升高(P<0.05)。相关分析结果显示,肝、肺、肾、小肠组织中TREM-1基因表达分别与TNF-αmRNA表达、ALT,Cr,MPO及DAO水平显著相关。结论脓毒症可导致多种组织TREM-1 mRNA表达增加,其改变与多器官损害密切相关。Objective To investigate the relationship between gene expression of TREM - 1 and multiple organ dysfunction in sepsis mice. Methods Sixty male mice were randomly divided into three groups as follows : normal control group ( n = 10) ,sham operation group ( n = 10) and cecal ligation and puncture (CLP) group ( n = 40). Animals were sacrificed at various intervals, and TREM - 1 mRNA expression in liver,lungs, kidneys and small intestine were detected by reverse -transcription PCR. In addition, serum ALT and Cr levels, pulmonary myeloperoxidase (MPO), and intestinal diamine oxidase (DAO) activities were also measured. Results The expression levels of TREM - 1 and TNF - α mRNA significantly increased in lung, hepatic and kidney tissues during 6,12,24 h after CLP as compared with control group ( P 〈0. 05 or P 〈0. 01 ), while there was no significant increase in intestine ( P 〉 0.05 ). Meanwhile, tissue TREM - 1 mRNA expression was significantly correlated with local TNF-α mRNA expression and organ function parameters in liver, lungs, kidneys as well as small intestine, including ALT, Cr, MPO and DAO ( P 〈 0. 01 ). Conclusion Sepsis can lead to remarkable up - regulation of TREM - 1 mRNA expression in vital organs, which might play an important role in the de- velopment of sepsis and subsequent multiple organ dysfunction syndrome.
