HIV抗体阳性孕产妇母婴阻断措施实施情况分析

摘　　要：目的了解商丘市预防艾滋病病毒母婴传播措施的实施情况及母婴阻断效果,探讨预防艾滋病病毒母婴传播对策。方法对2005-06/2007-07发现的48例HIV抗体阳性孕产妇进行问卷调查及随访,收集其妊娠结局、抗病毒药物服用情况、分娩方式、婴儿喂养方式、婴儿HIV感染情况等资料并进行分析。结果23例孕妇实施人工流产,2例自然流产,4例处于妊娠期,19例已分娩。分娩的产妇有84.2%规律服用过抗病毒药物,47.4%采用了剖宫产;19例新生儿中,89.5%规律服用过抗病毒药物,94.7%采用人工喂养。10例婴儿在满18月龄时检测HIV抗体为阴性。结论现行的预防艾滋病母婴传播措施是可行的。完善妇幼保健服务网络,提高孕期保健率,促进孕产妇HIV咨询检测工作,及时发现HIV感染孕产妇以及加强对阳性育龄妇女的随访是深化母婴阻断工作的重要途径。Objective To investigate implementation condition and effect of intervention to prevent HIV mother- to-child transmission, and to explore corresponding strategies. Methods A totall of 48 HIV-infected pregnant women identified from June 2005 to July 2007 were surveyed with questionnaire and followed up. Data on pregnant outcome, antiretroviral therapy （ART） for mothers and infants, modes of delivery, feeding practices and infection rate of infants were collected and analyzed. Results Except 4 women still in pregnant stage, 23 pregnant women terminated their pregnancies by artificial abortion, and 2 women got spontaneous abortion. There were 19 women delivered their babies and of them 84. 20% received ART, 47.4% chose cesarean section. The rate of receiving ART and formula feeding in 19 infants was 89.5% and 94.7% respectively. There were 10 infants had been tested HIV negative at the end of 18th months. Conclusion The intervention for PMTCT was effective and feasible. The important ways for PMTCT are developing health network for women and children, increasing the visiting rate in pregnant women, improving the rate of HIV counselling and testing, discovering HIV-infected pregnant women in time and following up all HIV-infected reproductive women.
