摘 要:目的研究畸变产物耳声发射(DPOAE)在早期诊断放疗后耳蜗功能损害时的作用。方法利用畸变产物耳声发射分析仪在放疗前、放疗中、放疗结束时、放疗结束后3个月对临床确诊为鼻咽癌、上颌窦癌患者的32例耳行畸变产物耳声发射(DPOAE)测试及纯音测听、声阻抗检查,比较耳声发射与纯音测听检测值在各频率的变化。结果放疗前耳蜗及中耳功能良好者,放疗中的畸变产物耳声发射幅值与放疗前1kHz〔(10±3.2)dB〕,2kHz〔(12±5.9)dB〕比较无统计学意义(P>0.05);但与放疗前4kHz〔(11±1.8)dB〕、6kHz〔(15±6.6)dB〕和8Hz〔(18±5.3)dB〕比较,有统计学意义(P<0.05)。结论畸变产物耳声发射(DPOAE)可用于鼻咽癌、上颌窦癌及垂体瘤等头颈部肿瘤放疗后耳蜗损伤早期诊断的检测。Objective To study the early diagnosis of Distortion Product Otoacoustic Emissions (DPOAE) of cochlear injury after radiotherapy (RT). Methods 32 ears in patients with Nasopharyngeal Carcinoma (NPC) or maxillary sinus carcinoma were included in the study. The pure tone audiometry, acoustic impedance and DPOAE were performed before RT, in the RT, after RT and 3 months after RT. The changes of each frequency were compared in the pure tone audiometric and DPOAE. Results DPOAE in the radiotherapy was changed after RT in the ears with good middle ear and cochlear functions. There were no statistically significant than those before RT at 1 kHz [ ( 10±3.2 )dB ] and 2kHz [ ( 12 + 5.9) dB ] ( P 〉 0. 05 ), but there were statistically significant in those frequencies at 4knz [ ( 11 + 1.8) dB ], 6knz [ ( 15±6.6) dB ] and 8 nz [ ( 18 + 5.3 ) dB ] ( P 〈 0. 05). And the changes of DPOAE were earlier than that of pure tone audiometric. Conclusion DPOAE can be used in early diagnosis of cochlear iniurv after radiotheranv.
