摘  要:目的评价超声血管增强技术(VET)在外周及颅脑血管疾病的临床应用价值。方法对10例正常志愿者及21例疑为颈部及脑血管疾病患者应用超声血管增强技术,显示正常及病变的血管,并与传统二维及彩色多普勒成像相对比。结果应用VET技术可更清晰显示10例正常颈动脉、股动脉内中膜及管腔,提高管腔及血管壁的清晰度,降低伪像效应。常规二维显像均无法显示指端小动脉及颅内血管管腔,应用VET技术清晰显示指端小动脉及其分支,9例可不同程度地显示Willis环大血管管腔。15例颈动脉斑块患者应用VET技术后可明显廓清斑块轮廓;4例脑动脉狭窄患者二维显像均无法显示狭窄血管,VET可显示血管病变处管腔局部变窄,随访观察其中2例支架置入术后患者,可清晰显示管腔内支架回声。2例脑动静脉畸形显示病灶处呈不规则液性暗区。结论超声血管增强技术可提高微血管显示能力和血管显示灵敏度,更清晰显示血管腔及管壁结构,具有较高的临床应用价值。Objective To evaluate the application of vascular enhancement technology (VET) to extracranial, peripheral and cerebral vascular diseases. Methods Ten volunteers and 21 patients with suspected carotid artery plaques and cerebrovascular diseases underwent VET diagnosis. Normal and diseased vessels were detected using vascular enhancement technology and were compared with traditional two-dimensional and color Doppler ultrasonography. Results Using VET, inter-media membrane and lumens in ten normal carotid and femoral arteries were displayed more clearly and the artificial effect in the lumen was lowered obviously. Finger tip arteries were not able to be found on 2-D ultrasonography but were clearly displayed even with small branches on VET. Arteries of Willis could be shown more or less on VET in nine volunteers, however, none of them could be shown on grey scale ultrasonography. Under VET scanning, carotid artery plaques in 15 cases were shown clearly the contour and outline, even the minimal thickness of anterior wall. Four cases with cerebrovascular stenosis could be diagnosed on VET and local lumen stenosis could be seen. In follow-up study, the lumen and hyperechoic stents could be visualized on VET in 2 patients after percutaneous catheter stent insertion. Irregular echoless area by VET indicated cerebral arteriovenous malformation in 2 cases and DSA confirmed the diagnosis. Conclusions Vascular enhancement technology can improve the display ability and sensitivity of micro vascular textures in periphery, extra-and intracranial cerebral vessels.
