摘 要:目的探讨速度向量成像(velocity vector imaging,VVI)在胎儿超声心动图中应用的方法学及可行性。方法20例胎儿取得标准胎儿四腔心观后,通过AxiusTMVVI工作站处理获得左室四腔心观心内膜VVI图,分别取得左室纵向6个节段的速度图、strain、strainrate及位移等曲线。6个节段间达峰时间(TPK)比较采用t检验。结果所有20例胎儿均获得清晰满意的四腔心左室VVI图,左室6个节段之间的速度、strain、strain rate及位移的TPK比较差异无显著意义(P>0.05)。速度分布呈现明显梯度特点,即从基底段向心尖,速度逐渐减小。VVI尚同时获得任意节段任意时间的速度、strain、strain rate及位移曲线及其三维图。结论VVI技术是新的室壁多方位动力学分析的定量技术,能够提供多种分析工具及获得多个参数,能够用于胎儿左室纵向室壁运动分析。Objective To assess the feasibility of velocity vector imaging (VVI) evaluation of left ventrieular dynamics in the fetuses. Methods Nineteen normal fetuses and 1 fetus with intermittent atrial extrasystoles were studied. The characteristics of VVI maps and multiple parameters of velocities,strain,strain rate and displacement of fetal left ventricles on four-chamber view in 20 fetuses were subsequently analysed using a novel AxiusTM VVI eehoeardiographie workstation. Results VVI examinations were acceptably reproducible in all 20 fetuses, time to peak (TPK) of velocities,strain,strain rate and displacement of 6 segments in LV were compared segment to segment, there were no significant differences among 6 segments. Conclusions Our preliminary investigation has demonstrated VVI evaluation of the fetal heart is feasible and reproducible.
