摘 要:目的心肌缺血/再灌注早期给予L-精氨酸,观察其对心脏的保护作用。方法雄性Wistar大鼠45只,随机分为3组(n=15):伪手术组(Sham组),缺血/再灌注+生理盐水组(Vehicle组),L-精氨酸早期处理组(L-Arg组)。结扎大鼠心脏左冠脉前降支,建立在体心肌缺血/再灌注模型,缺血30 min、再灌注5 h,分别在缺血/再灌注早期和晚期给予L-精氨酸。动态监测大鼠左心室功能;再灌注结束后,以Evan′s蓝和TTC双染法检测心梗面积,化学发光法检测总NO(NOx)含量,以ELISA法检测心肌组织过氧亚硝基(ONOO-)标志物硝基酪氨酸含量。结果再灌注早期给予L-精氨酸显著减轻了缺血/再灌注诱导的大鼠心功能降低(LVSP和dp/dtmax)、减少了心肌梗死面积、减少了心肌组织细胞毒性物质ONOO-的生成。结论心肌缺血/再灌注早期给予L-精氨酸可以通过增加NO的生成,减轻再灌注诱导的心肌损伤,进而加快心肌缺血/再灌注后心功能的恢复。Objective To determine the effects of L-arginine on the cardiac function of ischemic and reperfused myocardium of rats in vivo at early reperfusion, and to investigate the mechanisms involved. Methods Male adult rats were randomized to three groups, including Sham MI/R group, MI/R + Vehicle group and MI/R + L-arginine group. The rats were subjected to 30 minutes of myocardial ischemia followed by 5 hours of reperfusion. L-arginine was administered as a bolus at either 10 minutes before and 50 minutes after reperfusion. The cardiac function of left ventricle was continuously measured, and the infarct size, total NO and nitrotyrosine content in myocardium were detected. Results Administration of L-arginine at early stage of reperfusion markedly decreased myocardial infarct size, inhibited ONOO- and improved cardiac function of left ventricle. Conclusion At early reperfusion, cardiac function was improved by NO's inhabiting myocardial injury.
