血管镜辅助下内膜切除术治疗长段股-腘动脉闭塞性病变的护理

摘　　要：目的探讨血管镜辅助下半闭合式内膜切除术治疗长段股-腘动脉闭塞性病变的临床疗效及护理措施。方法甲顾分析了35例血管镜辅助下半闭合式内膜切除术治疗长段股-腘动脉闭塞性病变患者的临床资料。结果35例半闭合式内膜切除术技术成功率为100％，其中5例同时行股深动脉扩大成形术，3例行髂动脉球囊扩张及支架植入术，1例行髂外动脉内膜切除术。随访1～24个月，平均12个月，累积通畅率为85．7％，保肢率为100％。结论血管镜辅助下半闭合式内膜切除术治疗长段股-腘动脉闭塞性病变是安全有效的，术前、术后实施合理的护理是保证治疗成功的关键。Objective To discuss the clinical effect and nursing intervention of angioscopy-assisted semi-closed endarterectomy in treatment of extensive femoro-popliteal atherosclerotic occlusive disease. Methods The clinical data of 35 patients receiving angioscopy-assisted semi-closed endarterectomy for treatment of extensive femoro-popliteal atherosclerotic occlusive disease were retrospectively analyzed. Results The technical successful rate for semi-closed endarterectomy was 100%. Among the 35 patients, 5 had extended profundoplasties,3 had iliac arterial angioplasties with endovascular stenting and 1 had external iliac arterial endarterectomy . The patients were followed up for 1-24 months, with a mean of 12 months. The cumulative patent rate was 85.7% （30/35） and the limb salvage rate was 100%. Conclusion It is safe and effective to use angioscopy-assisted semi-closed endarterectomy for patients with extensive femoropopliteal arterial occlusive disease. Appropriate perioperative nursing care is the key for successful treatment.
