新生儿胃液乙肝标志物与宫内感染关系的研究

摘　　要：目的:探讨新生儿外周血及胃液HBeAg、HBsAg等标志物与乙型肝炎病毒宫内感染的关系。方法:连续收集母亲外周血标本和新生儿外周血及胃液标本,ELISA检测HBeAg、HBsAg,PCR方法检测HBV DNA;以ABBOTT试剂检测新生儿股静脉血的HBsAg。采用x^2检验、精确检验等统计学方法对结果进行分析。结果:共收集133例母亲、新生儿血标本和胃液标本,5例新生儿发生宫内感染;133例新生儿中单纯胃液HBsAg阳性有58例,单纯胃液HBeAg阳性9例,胃液HBsAg和HBeAg同时阳性7例,胃液HBeAg、HBsAg均阴性73例。PCR检测显示.胃液标本HBV DNA均阴性。统计学检验显示新生儿胃液HBsAg与乙肝病毒宫内感染有关系,而胃液HBeAg与乙肝病毒宫内感染无关系。结论:新生儿胃液HBsAg与乙肝病毒宫内感染有关,但是由于所有标本HBV DNA均阴性,尚不支持乙肝病毒宫内感染的胃液途径。Objective： To estimate the risk of Hepatitis B Virus intrauterine caused by HBsAg or HBeAg in neonatal gastric juice. Methods： Gastric juice Samples from neonates and serum samples from both mother and neonate were consecutively collected. The HBsAg and HbeAg in these samples were detected by ELISA, and HBV DNA were detected by PCR, The HbsAg in the femoral vein blood of neonates was also detected by ABBOTT agents. All data were analyzed by Chi square test and Fisher＇s exact test. Results： A total of 133 neonatal gastric juice were collected and 58 of were detected as positivity for HBsAg. 9 cases gastric juice were detected HBeAg positivity. 7 cases gastric juice were detected HBeAg and HBsAg positivity. 73 cases gastric juice were detected HBeAg and HBsAg. All cases gastric juice were detected HBV DNA negative by PCR. Statistic analysis indicated that association of HBsAg in gastric juice to HBV intrauterine transmission was significant. Conclusions： Although HBsAg in gastric juice was associated to HBV intrauterine transmission, HBV DNA was not detected in gastric juice, as a result, neonate HBV Intrauterine Transmission by food-born routine was not conclusive.
