Assessment by Echocardiography of Left Ventricular Diastolic Function in Patients with Hypertension Using the Atrioventricular Plane Displacement

摘  要:目的:探讨测量房室(AV)平面位移评价高血压病左室舒张功能的可靠性。方法:将82例轻中度高血压病患者按不同年龄分组,并设立相应正常对照组,应用M型超声心动图测量舒张期AV平面位移进行对比观察。结果:高血压病组左房收缩引起的AV平面位移及其与全舒张期位移的比值(A—AVPD/T—AVPD)显著大于对照组(P均<0.01),该比值与多普勒二尖瓣血流A/E存在良好的相关性(r=0.81,P<0.001),如以A—AVPD/T—AVPD≥45%判定高血压病左室舒张功能受损,具有较高的敏感性(85%)和特异性(90%)。结论:超声心动图测量A-AVPD/T-AVPD可用于评价高血压病左室舒张功能,尤其是检测左室松弛性受损。Objective: To assess left ventricular diastolic function in patients with hypertension using the artrioventricular plane displacement. Methods: The diastolic atrioventricular (AV) plane displacement was determined by echocardiography in 82 patients with mild to moderate hypertension and 53 age- matched normal controls. Results: Mean AV plane displacement resulted from left atrial systole and its ratio to the total diastolic displacement were significantly greater in hypertension than those in normal controls(P<0.01 for both). Ratio of motion of the plane caused by left atrial systole to the total diastolic displacement was correlated well with late- to - early mitral flow peak velocity index (r=0.81, P<0.001). Selecting a value of this ratio of AV plane displacement ≥45% to define Dopplor characteristics indicating impaired left ventrieular dysfunction, resulted in a high sensitivity(85%) and specificity(90%). Conclusion: Echocardiographic assessment of AV plane displacement can be used for the estimation of left vcntricular diastolic function in patients with hypertension and particularly in detecting impaired left ventricular relaxation.
