“助孕3号方”对早孕先兆流产患者Th1/Th2细胞因子和P、β-HCG影响的研究

摘　　要：目的:观察助孕3号方对早孕先兆流产患者Th1/Th2细胞因子、人绒毛膜促性腺激素和孕激素的影响,从免疫-内分泌调控角度探讨其安胎机理。方法:①病例:30例确诊为早孕先兆流产,辨证为脾虚、肾虚、或脾肾亏虚的妇女作为治疗组;20例正常妊娠的孕妇作为对照组。治疗组服助孕3号方治疗4周。②观察指标:外周血中Th1型细胞因子白细胞介素2(IL-2)和Th2型细胞因子IL-10(三色流式细胞标记法)、孕酮(P)、β-人绒毛膜促性腺激素(β-HCG)的浓度(ELISA法)。结果:①治疗组治疗前与对照组比较;IL-2显著升高(P<0.01),IL-2/IL-10比值显著偏高(P<0-01);②治疗组治疗后IL-2显著降低(P<0.01),IL-10显著升高(P<0.01),IL-2/IL-10比值显著下降(P<0.01);P、β-HCG显著升高(P<0.01);P的变化与IL-10的变化成显著正相关(P<0.05),β-HCG的变化与IL-2的变化成显著负相关(P<0.05)。结论:助孕3号方安胎机理在于:抑制Th1型细胞因子分泌,促进Th2型细胞因子分泌,纠正Th1/Th2细胞因子平衡的病理偏移;提高人绒毛膜促性腺激素、孕激素水平;且上述两方面作用机制存在一定相关性。Objective：To study the clinic effects of Prescription Zhuyun-Ⅲ（ZYⅢ） on early pregnancy women diagnosed as threatened abortion,and its mechanism in immunity and endocrine by determining the changes of serum Th1 and Th2-type cytokines,progesterone（P） and β-human chorionic gonadotropin（β-HCG）.Methods：The treatment group comprised of 30 early pregnancy women diagnosed as threateded abortion as well as deficiency syndrome of deficiency syndrome of Spleen or Kidney diagnosed by TCM,the control group consisted of 20 normal early pregnancy women at similar gestational age.The treatment group was administered with ZYⅢ for 4 weeks.Peripheral blood samples from the above two groups pre-and post-treatment were collected,serum Th1 type cytokine IL-2 and Th2 type cytokine IL-10 were determined by flow-cytometry.Serum P and β-HCG were determined by ELISA.Results：①The serum IL-2 level and the ratio of IL-2/IL-10 in the treatment group before therapy were significantly higher than the control group（P〈0.01）.② In the treatment group after therapy,the serum IL-2 level was significantly decreased（P〈0.01）,and the serum IL-10 level was significantly increased（P〈0.01）,so the ratio of IL-2/IL-10 was significantly decreased（P〈0.01）.Both P and β-HCG were significantly increased（P〈0.01）,and changes of P positively correlated with changes of IL-10（P〈0.05）,while changes of β-HCG negatively correlated with changes of IL-2（P〈0.05）.Conclusion：Our study suggest that ZYⅢ have evident function of fetus protecting,and its mechanism is：①inhibiting secretion of Th1-type cytokines,promoting secretion of Th2-type cytokines,and recovering pathological shift of Th1/Th2 balance.②increasing serum P and β-HCG concentrations.Moreover,there are some correlations between the above two effects.
