复方厄贝沙坦对高血压病并阵发性心房颤动及其P波离散度的临床干预

摘　　要：目的观察复方厄贝沙坦对高血压病并阵发性心房纤颤(简称房颤)及其P波离散度(Pd)的影响,探讨其对房颤预防及治疗的作用。方法将80例高血压病并发房颤患者随机分为治疗组和对照组,每组40例,治疗组给予复方厄贝沙坦治疗,对照组给予非血管紧张素转化酶抑制剂(ACEI)类及非血管紧张素受体拮抗剂类降压药治疗。治疗前、治疗后6个月均测量患者收缩压(SBP)、舒张压(DBP)及心率(HR),检测心电图(ECG)中的最大P波时限(Pmax)、最小P波时限(Pmin)、Pd(Pd=Pmax-Pmin)及超声心动图(UCG)中的左房舒张末内径(LADd)、左室收缩末内径(LVDs)、左室舒张末内径(LVDd)、室间隔厚度(IVST)及左室后壁厚度(LVPWT),计算左室肌重指数(LVMI)及射血分数(LVEF),并采用统计学方法,对其治疗前后各种指标进行比较。结果2组治疗后SBP、DBP、Pmax、Pd、LVPWT、LVMI均明显降低(P<0.01或0.05),同时治疗组LADdI、VST也明显降低(P<0.01或0.05),治疗组各项指标(除血压外)较对照组降低更明显(P<0.01或0.05),治疗组的房颤发生次数也显著低于对照组(P<0.05)。结论复方厄贝沙坦不仅能降压,改善高血压病并阵发性房颤患者左房功能,而且能降低房颤发生次数。Objective It is to observe the effect of Irbesartan/Hydrochlorothiazide combination on essential hypertension （EH） patients with paroxysmal atrial fibrillation （AF） and P wave dispersion, so to explore the clinical intervention and treatment function on AF. Methods 80 EH patients with paroxysmal AF were randomly divided into two groups： treatment group（40） and control group（40）. Treatment group was treated with Irbesartan/Hydrochlorothiazide combination and control group was treated with drugs such as ACEI or ARBs. Systolic blood pressure （SBP）, diastolic blood pressure（DBP）, Heart rate（HR）, Pmax, Pmin, Pd, LADd, LVDs, LVDd, IVST, LVPWT, LVMI, LVEF were measured and compared before treatment and 6 months＇ after treatment. Results After treatments all the parameters of SBP, DBP, Pmax, Pd, LVPWT, LVMI were decreased both in treatment group and control group（P〈 0.01 or P 〈 0.05）. The parameters of LADd and IVST were also decreased in treatment group（P 〈 0.01 or P 〈 0.05）. Compared with control group, all the pa- rameters except BP decreased more in treatment group（P〈 0.01 or P 〈 0.05）. The was lower than that in control group（ P 〈 0.05）. Conclusion of AF in treatment group combination not only can decrease blood pressure and improve left atrial function, but also prevent the reoccurrence of AF in EH patients with paroxysmal AF.
