摘 要:目的检测尖锐湿疣(CA)感染组织中人乳头瘤病毒(HPV)6型E7蛋白的表达情况及病毒复发转归的意义。方法152例的CA患者组织采用聚合酶链反应荧光法及S-P免疫法对初发和复发CA患者的活检组织和健康对照组织进行检测,HPV6型E7早期蛋白主要位于细胞核内呈棕黄色伴少许胞质着色为阳性,阳性率大于或等于15%者为阳性组织。结果152例CA患者中有91例HPV6型感染,感染率为59.9%。其中56例为初发患者,35例为复发患者;18~38岁53例,38~48岁26例,48~60岁12例;男52例,女39例。56例初发患者HPV6型E7蛋白阳性表达率为69.5%,35例复发患者的活检组织HPV6型E7蛋白阳性表达率为71.4%。27例正常宫颈上皮组织中HPV6型E7蛋白阳性率为0%,HPV6型E7蛋白在初发CA组织与正常宫颈组织比较以及复发CA组织与正常宫颈组织比较差异有统计学意义(P〈0.001)。初发与复发CA组织相比较率差异无统计学意义(P〉0.05)。HPV6型E7蛋白在不同年龄段阳性表达率分别为:53例18~38岁患者的阳性率为71.7%,26例38~48岁患者的阳性率为69.2%,12例48~60岁患者的阳性率为66.7%,不同年龄段阳性率差异无统计学意义(P〈0.05);52例男性患者中阳性率为75.0%,39例女性患者中为64.1%,阳性表达与性别无关(P〉0.05)。结论HPV6型是CA的主要致病因素之一,HPV6型早期蛋白E7在HPV6型感染的CA初发和复发组织中的表达和在正常生殖上皮组织中的表达差异有统计学意义,和年龄、性别无关;HPV6型早期蛋白E7的含量与其病情有着必然联系。Objective To analyze the expression of HPV6-E7 oncoprotein in condyloma acuminatum (CA), so as to explore its significance of viral relapse surveillance. Methods Fluorescence PCR was applied to detecting the expression of HPV6-E7 oncoprotein in 152 cases of CA tissues, and while S-P immunoassay was applied to determining tissues from incipient or relapsed CA patients and normal cervix uteri tissues. HPV6-E7 early protein was mainly located in cell nucleus, claybank dyed and accompanied with a little plasma colouring. The tissue with a positive rate of 15% or above was regarded as positive one. Results In 152 patients with condyloma acuminatum, 91 cases (59.9 %) were infected by HPV6.56 cases suffered from incipient condyloma acuminatum and 35 cases were patients with replased condyloma acuminatum. Age distribution: age 18-38, n=53; age 38-48, n=26;age 48-60, n=2. Sex distribution: males, n = 52;females, n= 39. The positive expression rates of HPV6-E7 oncoprotein was 69.5 %, 71. 4% in 56 cases of incipient CA tissue and 35 cases of relapsed CA tissue respectively, much higher than that of 27 cases of normal cervix uteri epithelial tissue (0%), P〈0. 001. The difference of HPV6-E7 oncoprotein expression wasn't significant between incipient and relapsed CA tissue (P〈0.05). The HPV6-E7 positive expression rates in different age period were as follows: age 18-38, 71.7%; age 38-48, 69.2%; age 48-60, 66.7%. The HPV6-E7 positive expression rates were 75.0% and 64.1% in 52 males and 39 females respectively. There was no statistically difference in different age period as well as in different sex. Conclusion HPV6 is one of the main etiological factors. There is statistical difference of the positive rate of HPV6-E7 oncoprotein between incipient, replased condyloma acuminatum tissues and normal germinal epithelial tissue. The expression of HPV6-E7 oncoprotein isn't correlative to age or sex. The content of HPV6-E7 oncoprotein is closely associated with the pathogenetic condition of condyloma acu
