切开复位接骨板内固定治疗掌侧Barton骨折19例临床分析

摘　　要：目的探讨桡骨远端Barton骨折的手术方法及疗效观察．方法19例闭合掌侧Barton骨折，所有病例均为手法复位失败者．均采用掌侧入路，使用普通接骨板12例，国产锁定接骨板7例．结果19例在6—18月后得到随访，骨折全部愈合．X线片示桡骨远端的掌倾角和尺偏角均恢复至正常范围，桡骨远端关节面恢复程度好．综合评分优17例，占73％；良2例，占23％．优良率达100％．出现环指伸肌腱卡压1例，取出此螺钉后症状消失．结论桡骨远端Barton骨折采用掌侧接骨板内固定治疗，可以较好地维持骨折复位，允许术后早期功能锻炼，并可获得满意的腕关节功能恢复．Objective To research the operative methods and the function of open reduction with internal fixation in the treatment of volar Barton＇s fractures of the distal radius. Methods 19 cases of volar closed Barton＇s fracture involved in this study. The fractures were fixed with the ordinary plate in 12 cases and locking plate in 7 cases, the volar approach of the distal radius was used. Results 19 cases were followed up 6 - 18 months, all the fractures healed well. The roentgenograms showed that the volar tilt and the ulnar inclination were well maintained. Results of evaluations according to functional criteria were excellent in 17 cases, accounted for 73%, good in 2 cases, acccounted for 23% , the excellent and good rate reached 100%. One patient was associated a locked extensor digitorum tendon by screw. Conclusion Open reduction and internal fixation of volar Barton＇s fracture can restore articular congruity and result in good to excellent function to the wrist.
