摘  要:目的探讨新生猪急性缺氧缺血脑水肿早期MRI的改变、相应脑组织水通道蛋白4(AQP4)的表达及二者的关系。方法选择生后3d的健康新生猪14只,随机分成实验组和对照组(假手术组),在缺氧缺血后0、1.5、3、4.5和6h进行MRI扫描,获得明显脑水肿区域的表观扩散系数(ADC)、磁共振扩散加权成像(DWI),应用免疫组织化学方法观察缺氧缺血后6h脑组织AQP4的表达。结果实验组在缺氧缺血后1.5h时,脑DWI图像上开始出现高信号区,局部ADC值下降,主要分布予大脑皮层、基底神经节及脑室旁区,在3h和6h的DWI图像上,高信号区更加明显且范围扩大,ADC值明显下降,6h后牺室旁的AQP4表达明显增多,相对比表达强度(吸光度值)为(15.0±4.0)%,明显高于对照组(5.1±2.9)%,差异有显著性(t=3.09,P〈0.01),并与该区域的ADC值呈显著负相关(r=0.876,P〈0.01),与rs-DWI(DWI高信号区面积与同层脑总面积比值)呈显著正相关(r=0.846,P〈0.01)。结论ADC、DWI能够早期诊断缺氧缺血性脑水肿,AQP4在急性缺氧缺血脑水肿的发生机制中起重要的作用,ADC、DWI可间接反映AQP4的表达水平。Objective To investigate MRI changes in the early stage of hypoxie ischemic brain edema, and to study Aquaporin-4 (AQP4) expression and their correlation. Methods Fourteen neonatal pigs were randomly divided into two groups: operation group (n=8) and pseudo-operation group (n=6, as control group). MRI were done at a different interval of time 0 h, 1,5 h, 3 h, 4.5 h and 6 h after hypoxie ischemie brain damage. Immunohistoehemical investigation of AQP4 expression was followed by MRI at 6 h after hypoxic ischemic brain damage, Results We observed a significantly decreased apparent diffusion coefficients (ADC) and an increased diffusion weighted imaging(DWI) within injured cortical and hippoeampus regions. There was a significant increase in AQP4 expression [(15.0±4.0)% vs (5.1±2.9)%] in the injured tissues(t=3.09, P〈0.01), and its correlation with regional MR changes was significant (r=0.846, P〈0.01). Conclusion ADC and DWI play an important role in detection of earlier brain edema. AQP4 may be involved in edema formation, and the changes of ADC and DWI may reflect the level of AQP4 expression.
