保留皮肤的美容切口腋臭根治术

摘　　要：目的 探讨如何在腋臭根治术中既能避免术后影响患者上肢功能，又能达到美观效果。方法 先沿腋窝中央皮肤褶皱处设计手术切口线，切开皮肤后，剪除腋毛区域及周围1cm皮肤上的脂肪组织、顶泌汗腺及毛囊，修剪成仅含皮肤全层和真皮下妞管网的超薄皮瓣，最后缝合切口结果72例皮瓣全部成活，术后切口瘢痕不明显，上肢活动自如．术区皮肤柔软，腋毛稀疏，腋窝不再有异味。3例患者术区愈合延迟，换药后愈合结论本手术切口小，术后切口瘢痕不明显，术区皮肤柔软，可达到患者美容要求。手术并不切除腋窝皮肤，术后患者上肢功能不受影响、而且手术在直视下破坏顶泌汗腺，手术彻底，操作简单，术后效果满意。本法缺点是术区毛囊破坏，腋毛脱落。Objective To study how to cure bromidrosis permanently with beautiful outlook without effecring the patients＇ upper limb function after operation. Methods To design the operative incision aleng the central armpit wrinkles, we cut the adipose tissue, apocrine sweat glands and hair follicle inside axillary hairs and outside for one centimeter after we cut the skin, Super thin skin flaps were formed, which only contain the whole skin and under - dermal blood vessel web. At last we sew up the incision, Results The flaps in 72 cases were all survived. The postoperative scars were not obvious. What＇ s more, the patients could exercise their upper limbs freely. There was not any strange flavor in the armpits. And the skins were soft with rare axiliary hairs. Three cases had delayed healing after dressing the wound. Conclusion The operation has a small incision, not obvious scar and a soft skin, which achieve the patients＇ cosmetic demands. The postoperative patients can exercise their upper limbs freely because of reserving the armpits skin. Besides the operation destroys the apocrine sweat glands under steady look. It＇ s not only thorough hut also simply with good results. The shortcoming of the operation is that the hair follicle are destroyed and the axillary hairs drop.
