倍频激光联合中药补阳还五汤治疗糖尿病性视网膜病变

摘　　要：目的 探讨532倍频激光光凝联合中药补阳还五汤治疗糖尿病性视网膜病变的·临床疗效。方法 Ⅲ～Ⅳ期糖尿病视网膜病变58例（116眼）随机分为对照组和治疗组，对照组单纯使用532倍频激光机进行光凝治疗；治疗组在进行光凝治疗同时联合中药补阳还五汤口服。随访3—12个月，观察患者的视力、眼底检查、眼底彩照及眼底荧光血管造影变化。结果 补阳还五汤治疗组患者视力提高或改善占88．3％，对照组为71．4％，补阳还五汤组优于对照组（P〈0．05）；补阳还五汤组黄斑水肿治疗有效率为90．9％，对照组为70．0％，补阳还五汤组优于对照组（P〈0．05）；两组在视网膜水肿、渗出、出血以及视网膜新生血管或无灌注区消退等方面，补阳还五汤组有效率为90．0％，对照组为83．9％，两组之间差异无统计学意义（P〉0．05）。结论激光联合中药补阳还五汤可有效地治疗糖尿病视网膜病变，补阳还五汤可改善光凝术后患者的视力，并促进黄斑水肿的吸收。Objective To investigate the treatment of diabetic retinopathy with combination of 532nm double frequency laser and buyang huanwu decoction（BYHWD）. Methods One hundred and sixteen eyes in 58 cases respectively suffering from diabetic retinopathy in phase Ⅲ-Ⅳ were divided into groups of control and treatment, randomly. Both of these two groups received the treatment of photocoagulation with 532nm double frequency laser. After that the treatment group was given BYHWD. The follow - up period was 3 - 12months, the visual acuity examination, color photograph and fundus fluorescein angiography （FFA） were performed in 3,6, 12 months after treatment. Results Of the BYHWD group, visual acuity was improved or maintained in 88.3%, while the control group was 71.4% ; The macular edema retrogression was 90.9% in the BYHWD group, 70.0% in control group. TheBYHWD group was better than the control group in the visual acuity and macular edema retrogression （ P 〈 0. 05 ）. After treatment hemorrhage, exudation and edema of retina were improved, new vessels and nonperfusion areas were disappeared completely or partly. The effective rate was 90.0% in BYHWD group, 83.9% in the control group, there was not significant difference between two groups （ P 〉 0. 05 ）. Conclusion The combination of laser photocoagulation and BYHWD is an effective method to treat diabetic retinopathy, BYHWD could protect visual acuity and improve macular edema retrogression.
