CT引导下经皮肺穿刺与活检针内径相关性的研究

摘　　要：目的探讨粗针(14G、16G)、细针(18G、20G)经皮穿刺活检肺部病变的阳性率及并发症的差异。方法对168例CT引导下经皮肺穿刺活检的病例资料进行回顾性分析,其中80例采用细针活检,88例采用粗针活检,比较两组穿刺结果的阳性率及并发症。结果粗针穿刺组中肺癌72例,阳性率为98.6%;良性病变16例,阳性率为81.3%。细针穿刺组中肺癌66例,阳性率81.8%;良性病变14例,阳性率64.3%。粗针穿刺组发生气胸9例,肺出血7例,并发症发生率18.2%;细针穿刺组发生气胸7例,肺出血4例,并发症发生率13.8%。结论粗针行CT引导下经皮穿刺活检阳性率有显著差异。Objective To evaluate the difference of diagnostic efficacy and complications between thick needle （ 14G, 16G） and thin needle （18G,20G） in transcutaneous lung biopsies under CTL-guided procedure. Methods Transcutaneous lung biopsies were performed using thick needle and thin needle in 168 cases respectively, 88 cases of which were punctured with thick needle and 80 cases of which were punctured with thin needle. Their diagnosis efficacy and complications were compared. Results In the thick needle group, 71 of 72 patients with lung cancer were accurately diagnosed by biopsies （98.6%）. 13 of 16 patients with benign lesions were diagnosed by biopsies as well （81.3%）. In the thin needle group, 54 of 66 patients with lung cancer were diagnosed by accurately biopsies （81.3%）. 9 of 14 patients with benign lesions were diagnosed by biopsies （64. 3% ）. Pnenmothorax was found in 9 cases and pulmonary hemorrhage was found in 7 cases in the thick needle group （ complication rate of 18.2% ）, of which only 1 case was handled with drainage and the others were not particularly treated. Pnenmothorax was found in 7 cases and pulmonary hemorrhage was found in 4 cases in the thin needle group, （ complication rate of 13.8% ）, of which no case needed special treatment. Conclusion There are significant differences between the two groups in diagnostic accuracy, but no differences between the two groups in the complication rate. Transcutaneous lung bi- opsies under CT-guided is worthy to be popularized because of its safety.
