摘  要:目的了解中国人群Kell血型中K+型的分子生物学特征,并研制序列特异性引物-聚合酶链(PCR-SSP)试剂盒,为制订血型检测策略提供依据。方法测序分析血清学鉴定K+标本,确定K+突变类型,根据突变类型分别设计K、k基因型的特异性引物,研制PCR-SSP扩增试剂盒,同时分析100名上海地区献血者、23名外籍献血者、72名新疆地区维吾尔族献血人群的K/k基因型,并随机抽取阳、阴性标本进行测序。结果血清学鉴定为K+的突变类型为C698T突变;经研制的K/kPCR-SSP试剂盒可以特异性扩增K基因和k基因。1例新疆标本基因型为K+k+;3例外籍献血者标本基因型为K+k+,其余均为K-k+。结论本研究中所有中国人K+的分子生物学特征与西方人一致,经研制的PCR-SSP试剂盒可直观有效地检测KELK/k基因型。不同种族、不同地区献血者K+频率的差异有显著性,制订相关检测法规或条例时应有所考虑,建议外国人献血、产前检查时加做K+检测。Objective To know about the molecular basis of the K+ in Chinese and make the PCR-SSP kit to analyze the frequency of K+ genotype in 3 different donor groups. Methods The K+ phenotype samples positive controls were sequenced to check the mutation type in Chinese,then 2 pairs of specific primers for K and k genes were designed for this mutation and the PCR-SSP kit was made to detect K and k genes, then this kit was used to analyze three different donor groups including 100 Shanghainese, 72 Uigurs and 23 foreigners respectively. All the K+ samples and some K- samples were sequenced for confirming. Results The results showed that all the K+ mutation in our study was C698T type,just the same as reported. The kit we made was useful in detecting the K gene and the k gene. Among three groups, 1 Uigur and 3 foreigners were found as K+ with the genotype of K+k+ ,while the others were K-k+. Conclusions All the Chinese K+ in this study have the same molecular basis as the most caucasion K+ ,and th': kit we made is specific and useful. Different population from different ethnicitles or regions varied with different frequency of K+gene. For the foreigners living in China, we suggested the regulations about transfusion relating tests or prenatal tests should consider the Kell blood type.
