摘 要:目的 研究不同浓度胆红素下干化学法和湿化学法对肌酐测定的影响,探讨两者检测结果的差别与胆红素浓度的线性关系及相关程度。方法 于2007年1~4月取临床标本51例,分别采用干化学法和湿化学法(碱性苦昧酸法)测定每例标本肌酐,对结果进行统计学分析。结果 胆红素浓度低于104.8μmol/L时,干化学法和湿化学法对测量肌酐的差异无显著性。胆红素浓度〉111.2μmol/L时,干化学法和湿化学法测量肌酐的差异有显著性(P〈O.01)。两种方法测得结果差值与胆红素浓度有较好的相关性,相关系数r=0.8728,回归方程Y=-1.906+0.1329X。结论 低胆红素浓度下,干化学法与湿化学法均可作为肌酐测定的常用方法。高胆红素浓度下,两种方法偏差较大,可考虑用酶法测定肌酐或先消除胆红素再行测定。Objective To investigate the effect of the dry and wet chemical methods for detecting creatinine in different bilirubin concentration, and to explore the linear relationship and the related degree between the differences of two effects and the bilirubin concentration. Methods The experiment was carried out from January to April 2007 in the biochemistry room of the clinical laboratory section of the Xijing Hospital. 51 clinical specimens were selected. (1) To determine the creatinine of every specimen by dry chemical methods. (2) To determine the creatinine of every specimen by wet chemical methods-alkaline picric. (3) To handle these results with statistic methods,and to analyze the correlation among them. Results The measures of dry and wet chemical methods for detecting creatinine had no clear differences when the bilirubin concentration was down to 104.8μmol/L. But there was significant difference when the bilirubin concentration was up to 111. 2 /μmol/L (P〈0. 01). The difference of results got from the two methods had better correlation to the bilirubia concentration (r = 0. 872 9). The recurrence equation Y= -1. 906 + 0. 132 9 X. Conclusion The dry and wet chemical methods can both conducted as the common methods of creatinine determination in low bilirubin concentration, while the difference of the two methods is big in high bilirubin concentration,so it is necessary to further demonstrate by experiments or wipe off bilirubin.
