摘  要:目的探讨超生理雌孕激素水平在胚胎移植助孕后异位妊娠发生中的意义。方法对1997年9月至2006年10月在河北医科大学第二医院生殖医学科行新鲜胚胎和冻融胚胎移植获得妊娠407周期进行回顾性分析。结果新鲜胚胎和冻融胚胎移植周期异位妊娠率分别为7.45%、1.18%,差异有统计学意义(χ2=4.5958,P=0.0321)。新鲜周期中异位妊娠组与非异位妊娠组输卵管因素比例分别为79.2%、56.4%,异位妊娠组胚胎移植后3d血清孕酮[(120.1±32.3)nmol/L]明显高于非异位妊娠组[(89.7±43.7)nmol/L],差异均有统计学意义(P<0.05)。结论超生理雌孕激素水平及其比例失调可能与胚胎移植助孕后异位妊娠发生有关,并在异位妊娠的发生中与输卵管受损联合起作用。Objective To investigate the effects of supra-physiological estradiol(E2) and progesterone(P) levels in ectopic pregnancy(EP) after embryo transfer(ET). Methods A retrospective study of dO7 pregnancy Cycles was carried out after fresh embryo transfer and frozen-thawed embryo transfer(FET) from the center of Reproductive Medicine of the second hospital of Hebei Medical University from September 1997 to October 2006. Results The rate of EP in fresh ET cycles(7.45% ) was significantly higher than the corresponding value ( 1. 18% ) in FET cycles ( x^2 = 4. 5958, P = 0. 0321 ). In fresh ET cycles the ratio of tubal factor (79. 2% ) and the level of serum P[ ( 120. 1 ± 32. 3) nmol/L] on day 3 after ET in EP group were significantly higher( P 〈 0. 05) than those[ ( 89.7 ± 43. 7) nmol/L] in non-EP group. Conclusion Higher level of progesterone may be a possible cause of ectopic pregnancy after embryo transfer in supraphysiological ovarian steroid situation, combined with tubal damage for the etiology.
