摘  要:目的观察无创正压通气(NIPPV)治疗慢性阻塞性肺病(COPD)急性加重期并呼吸肌疲劳疗效。方法54例COPD患者随机分为治疗组(26例)和对照组(28例),比较治疗前后两组血气分析、呼吸频率、辅助呼吸肌动用评分、呼吸困难评分、肺功能变化。结果治疗组在无创正压通气治疗72h后,PaCO2降低,Pa02明显升高,呼吸频率下降,辅助呼吸肌动用明显减少,且肺功能明显改善,与治疗前及对照组相比,P<0.05,差异有统计学意义。结论COPD急性加重期患者虽无呼吸衰竭但多有呼吸肌疲劳,早期应用NIPPV能缓解呼吸肌疲劳,改善肺的通气和换气功能,缩短住院时间。objective To observe the clinical effect of NIPPV on respiratory muscle fatigue in patient with COPD without respiratory failure. Methods Fifty-four COPD patients with respiratory muscle fatigue were divided into two groups (therapy group and comparison group).The arterial blood gases, respiratory rate, dyspnea score , scale for accessory muscle use and outcome, and pulmonary functions were compared between the two groups. Results compared before treatment and comparison group.Therapy group showed improving PaO2, Lowering PaCO2 ,respiratory rate,dyspnea score, scale for accessory muscle use , FEVI%,FEVI FVC% were improved after NIPPV.(P〈0.05).The differences btween two groups being statistically significant .although respiratory failure does not occur in patients with COPD, respiratory muscle fatigue already exists. NIPPV interventin can decrease respiratory muscle fatigue and improve the ventilation and gas exchange of the lung, decrease the duration of hospital.
