摘 要:目的研究应用体外反搏技术,观察对结核性包裹性胸腔积液患者包裹范围大小、积液的体积以及胸膜肥厚程度的影响。方法选择包裹性胸腔积液患者200例,随机分为对照组及应用组,对照组给予正规抗结核治疗。应用组在正规抗结核治疗基础上加用体外反搏。两组患者均在体外反搏治疗前,疗程结束时及结束后30d,分别行胸部平片、B型超声及胸部CT检查,观察包裹积液范围大小、积液的体积以及胸膜肥厚程度的变化,计数资料用χ2检验,计量资料用t检验,对结果进行统计学分析。结果应用组:治愈19例,有效65例,总有效率84%。对照组:治愈7例,有效59例,总有效率66%。两组间有高度显著性差异P<0.01。结论体外反搏通过提高舒张期主动脉内舒张压,提高对多脏器的血液灌注,加快肺部及胸膜的血液循环。对结核性包裹性胸腔积液患者,能有效促进包裹中的液体吸收,减少其胸膜的肥厚、粘连、钙化,有肯定的效果。Objective Studying the using of external counterpulsation,to observe the clinical effects of the dimension of the capsual ,the effusion volume and the thickening of tuberculo-encapsulated pleural effusion patients. Methods Selected 200 encapsulated pleural effusion patients,randomed into the matched group and the applicaition group, treat the matched group with negularly anti-tuberculosis therapy. Treat the applicaition group with external counterpulsation besides regular antl-tuberculosis therapy, before being trated with external counterpulsation,when the course of treatment stops and after 30 days of the course of treatment, both of the two groups are in progress of plain chest film, B-mode ultrasonography and the chest CT examine,to observe the dimension of the capsual,the volume of the effusion and the change of the thickening of the pleurae, categorical data be tsted by chi-square test,measurement data be tsted by t test,analysis the result by statistics. Result The applicaition group:19 patients cured,65 patients improved,total efficiency 84%. the matched group 7 patients cured,59 patients improved, total efficiency 66%. The two groups have a notable difference (P 〈 0.01 ). Conclusion External counterpulsation technique can raise Aorta Diastolic pressure at phase of relaxation,can promote blood offusing of Many visceral organs, speed the blood circle of bellows and pleura. For tuberculo-encapsulated pleural effusion patients, external counterpulsation technique can effectively promotes the effusion ab- sorption from the capsule ,reduce the thlckening,adhesion ,calcification of the pleura, it has affirmative effects.
