反取中厚皮与全厚皮回植的对比研究

摘　　要：目的探讨使用反取皮回植法治疗皮肤撕脱伤的方法。方法应用鼓式取皮机反取中厚皮回植治疗92例,反取全厚皮回植治疗147例,对两组病例的回植成活情况进行对照研究。结果在撕脱面积不大时两组间成功率差异不明显,在撕脱皮肤面积较大时,反取中厚皮回植组成功率显著高于全厚皮回植组(P<0.05)。结论小面积皮肤撕脱伤时,全厚皮回植治疗效果较好;在撕脱皮肤面积较大时,采用反取中厚皮回植是一种更为理想的方法。Objective To investigate the management of skin avulsion by back grafting mesh flaps taken from avulsed skin.Methods The split thickness skin grafts were harvested from avulsed skin by using dermatone of drum type,and the mesh flaps were back grafted for 92 cases with skin avulsion,and 147 cases were treated with back grafted of full-thickness skin grafts.Compared the survives of back implantation between the two groups.Results While the area of the skin avulsion was small,the survival ratio between these two groups were almost the same.And while the area of the skin avulsion was large,the survival ratio in split-thickness skin graft group was higher than that of full-thickness skin graft group（P〈0.05）.Conclusion Full-thickness skin back grafting is more effective for the treatment of small size of skin avulsion;and split-thickness skin back grafting is better for large skin avulsion.
