摘 要:目的研究无外源性生长因子及细胞支架的作用下,应用体外培养传代的骨髓间充质干细胞(bonemesenchymal stem cells,BMSCs)与透明质酸钠混合溶液修复大鼠早期退变椎间盘的作用。方法取同种基因大鼠的BMSCs,通过贴壁法培养传代,与医用透明质酸钠混合配制。10只SD大鼠,尾椎上取相邻3个椎间盘,由近端至远端设为细胞植入组?椎间盘退变组?正常对照组。分别于细胞植入组、椎间盘退变组建立退变模型。1周后,细胞植入组注射BMSCs与透明质酸钠溶液。3周后,X线片比较椎间盘高度指数(disc height in-dex,DHI),组织病理学方法行病理观测并评分,RT-PCR测定Ⅱ型胶原和聚集蛋白聚糖的 mRNA表达。结果细胞植入组的DHI值与椎间盘退变组相比无明显改善且远低于对照组。病理切片结果提示植入组髓核纤维环的形态得到保留,髓核纤维环边界清晰,细胞产生增殖分裂,其病理学评分明显高于椎间盘退变组。RT-PCR结果提示细胞植入组Ⅱ型胶原和聚集蛋白聚糖的 mRNA表达水平较椎间盘退变组明显提高。结论在椎间盘退变早期植入BMSCs与透明质酸溶液可以延缓椎间盘的退变,而不需要外源性细胞因子和三维支架的作用。Purpose The primary aim of this investigation was to determine whether expanded BMSCs in vitro mixed with sodium hyaluronate could repair disc degeneration in an early stage in rats without the addition of exogenous growth and scaffolds. Methods Bone mesenchymal stem cells from syngenetic rats were cultured in vitro and mixed with sodium hyaluronate. The proximal three intervertebral discs in the tails of 10 SD rats, from near to remote, were served as cell transplanted group,disc degenerative group, and normal control group, respectively. Disc degeneration was induced in the cell transplanted group and disc degenerative group. After one week, the cell transplanted group was treated with the mixture of BMSCs and sodium hyaluronate. Then, the tails were harvested at 3 weeks, measurements of intervertebral disc height,histologic appearance were made and RT-PCR was performed to mesasure the mRNA expression of Type Ⅱ collagen and aggrecan. Results Radiographs showed that the normal control group kept the normal disc height index, the cell transplanted group and disc degeneration group were reduced. Histologic showed that the structure of the inner annulus fibrosus was significantly preserved in the cell transplanted group, and the boundary between the nucleus and annulus could be clearly visualized. The cells proliferated and made lots of matrix. Expression of mRNAs of the nucleus pulposus aggrecan and Type Ⅱcollagen was significantly greater in the cell transplanted group than in the disc degenerative group. Conclusions Transplantation of the mixture of BMSCs and sodium hyaluronate in the early stage of disc degeneration can postpone disc degeneration without the addition of exogenous growth and scaffolds.
