摘  要:目的:探讨细胞因子与骨代谢生化指标在老年骨质疏松症发病机制中的重要作用及相互关系。方法:采用放射免疫分析检测85例老年骨量减少和骨质疏松患者血清白细胞介素-6(IL-6)、肿瘤坏死因子α(TNF-α)和骨钙素(BGP)的水平和骨密度检测,同时与青中年和老年健康人比较。结果:发现老年骨质疏松患者骨吸收指标IL-6和TNF-α水平随病情加重而增高,与不同年龄对照组比较有显著性差异(P<0.05,P<0.01)。骨形成指标BGP则不同程度降低,均与青中年对照组有明显差异(P<0.05)。骨量减少组骨密度值明显低于青中年对照组(P<0.01),而老年骨质疏松组骨密度明显低于骨量减少组(P<0.05)。结论:骨质疏松患者从骨量减少到骨质疏松的病理过程中,骨转化呈现骨吸收增加而骨形成减少的变化规律。IL-6、TNF-α与BGP可作为骨质疏松症的观察指标和辅助诊断的项目。Objective To study the relationship between the serum levels of IL - 6, TNF - α and bone r - carboxyglutamic acid protein (BGP) in elderly subjects with osteoporosis. Methods Serum IL - 6, TNF - α and BGP levels were measured with RIA in 40 elderly patients with osteoporosis, 40 elderly subjects with decreased bone density but not to the degree of of osteoporosis ( i. e. decreased bone mass), 35 aged controls ( above 60) and 35 younger controls ( below 45). Results By definition, the bone density in patients with osteoporosis was significantly lower than that in all other groups ( P 〈 0.05 - 0.01 ) and the bone density in subjects with decreased bone mass only was also significantly lower than that in younger controls ( P 〈 0.01 ). The serum IL - 6 and TNF - α levels in patients with osteoporosis were significantly higher than those in all controls ( P 〈 0.05 - 0.01 ), especially in advanced cases, while the BGP levels were significantly lower. Conclusion Serum IL - 6, TNF - α and BGP levels could be used as indicators of the de- gree of osteopemsis.
