摘 要:目的:细胞凋亡(Fas)参与免疫调节及清除病毒,本文就Fas与慢性肝病中组织学活动指数、谷丙转氨酶及乙型肝炎病毒脱氧核糖核酸的关系作一分析,以期了解Fas在慢性肝病中的意义。方法:以Kiyoshi半定量法对49例慢性肝病患者肝组织的Fas表达进行了分析,部分患者进行了随访。其中慢性活动性肝炎(CAH)25例,肝硬化(LC)6例,原发性肝癌(HCC)18例。结果:25例CAH中18例占72%阳性,多呈弥漫型分布。门脉或门脉周围炎症重者阳性信号明显。ALT升高、HAI分值高者Fas呈高表达。在CAH细胞凋亡和坏死并存。原发性肝癌癌组织的Fas表达为片状或点状分布。INF治疗后肝组织Fas表达增虽。Aim:Fas and Fas ligand play an important role in regulating normal immune responese and viral clearance.The significance of Fas expression in chronic liver diseases was analyzied histologically along with HAI,ALT and HBV DNA in this study.Methodes:Fas expression was examined semiquantatively by methode of kiyoshi.The 49 cases of chronic liver disease,including 25 of CAH,6 of cirrhosis and 18 cases of HCC,seven cases of CAH treated by INF α2b,the expression of Fas were followed up.Results:of the 25 cases of CAH the Fas were detected positive in 18(72%),most of the Fas were of the diffusive type,the inflammatory signal were conspicuous around the portal and periportal zone.The ALT were elevated,HAI scored high and the expression of Fas were significant.The apoptosis and necrosis were coexisted in these cases of CAH,The Fas expression in the cases of HCC were of the circumscribed or spotted type.It was noted that Fas expression often enhanced after the INF treatment.Conclusions:Fasmediated apoptosis along with necrcsis phay an important role in the pathogenesis of viral hepatitis.
