摘 要:目的观察中药复合静脉制剂血必净注射液对兔心肌缺血再灌注损伤的保护作用。方法2~3kg健康家兔40只,随机分为5组,每组8只。假手术组(A组)、缺血/再灌注模型组(B组)、血必净术前一天给药组(C组)、术前连续两天给药组(D组)、术前连续五天给药组(E组);进行心肌缺血再灌注手术并分别记录旷置或结扎15min,再灌注60min、2h、4h心电图及心脏血流动力学指标。并于上述各时间点抽取左心室血1.5ml检测心肌肌钙蛋白I(cTn-I)、肌酸激酶(CK);留取血清检测丙二醛(MDA)、超氧化物歧化酶(SOD);术后留取心肌组织做HE染色观察组织结构及炎症反应情况。结果血必净各组(C、D、E组)家兔血清中cTn-I、CK、MDA含量均明显低于B组(P<0.05);SOD均高于B组(P<0.05);ST段抬高的程度呈下降趋势。结论血必净注射液能改善心肌局部血液循环,保护血管内皮细胞,减少心肌酶的释放,同时可降低MDA,提高SOD活性,对兔心肌缺血再灌注损伤有保护作用;且术前5d连续给药效果更佳。Objective To observe the effect of xue-bi-jing injection on ischemia-reperfusion injury of myocardium of rabbits. Methods Forty rabbits weighting 2 to 3 kg were divided randomly into 5 groups, each with 8 rabbits. Sham-operated group ( A), ischemia/reperfusion model group ( B), the group of giving xue-biojing injection the day before operation (C), the group of giving xue-bi-jing injection 2 days continuously before operation (D), the group of giving xue-biojing injection 5 days continuously before operation (E);do the operation of myocardial ischemia and reperfusion, record ECG and cardiac hemodynamic indicators of exclusion or ligation of 15 min,reperfusion 60 min,2 h,4 h. And above each time point,taking 1.5 ml of blood from left ventricle to detect cardiac troponin Ⅰ (cTn-Ⅰ) ,creatine kinase (CK) ; collecting serum to detect malondialdehyde (MDA) and superoxide dismutase (SOD) ;collecting myocardial tissue after operation to observe its structure and inflammatory reaction by HE stain. Results Rabbit serum cTn-Ⅰ, CK, MDA of group ( C, D, E) were significantly lower than the model group ( P 〈 0. 05 ) , SOD were higher than the model group (P 〈 0.05 ); ST elevation was the extent of the downward trend. Conclusions Xue-bi-jing injection can improve regional myocardial blood circulation, protect endothelial cells and reduce myocardial enzyme's release ,at the same time it can reduce the MDA, enhance the activity of SOD. So it can protect rabbit from myocardial ischemia-reperfusion injury and a preferable treatment effect for preoperative continuous medication.
