滑石粉治疗老年性气胸42例疗效观察

摘　　要：目的:探讨滑石粉治疗老年性气胸的疗效。方法:使用滑石粉疗法,先从胸腔引流管中一次性注入利多卡因100 mg局麻止痛,再用无菌滑石粉2 g溶解于生理盐水20 ml中,从胸腔引流管中一次性注入,让患者翻身使药液分布均匀。2周后若气胸不愈合,再给滑石粉治疗1次,最多不超过3次。结果:42例患者中,使用上述方法1次后,有33例气胸愈合,其中最短4天,4例用药2次后愈合,3例用药3次后愈合,其中最长1例116天,另2例气胸不愈合患者中1例转外科手术,1例死于肺部感染。结论:滑石粉治疗老年复发性气胸疗效明确,若能正确使用,并尽量减轻其不良反应,不失为治疗老年性气胸的一种好方法。Objective ：To investigate the effect of talcum powder in the treatment of geriatric pneumothorax. Methods：42 cases of geriatric pneumothorax from 2002 in our hospital were collected,among them,38 cases were male,4 cases were female aged 63 to 87 years with an average age of 73 years. All the patients had the medical histories of chronic cough and expectoration over 5 years, and were suggested with pulmonary emphysema and pneumonthorax by X-ray chest film. All of them received closed chest drainage plus negative pressure aspiration over 10 days.The patients whose pneumothorax was not cured were treated with talcum powder,First,lidocaine 100 ml was injected into the thoracic acvity for anesthesia,and talcum powder in 0.9% NS 20 ml was done via the thoracic drain at a time.After that, the patients were told to roll their body in order to scatter talcum powder well-distributedly. The above method would be performed again if their pneumotherax was not cured,but the frequency could not exceed 3 times. Results：Among 42 cases,33 eases were cured only once,4 cases were cured by two times,3 cases by three times;the shortest time of treatment was 4 days and the longest time was 116 days;another 2 cases were referred for surgical operation on account of their pneumothorax was not cured;only one died from pulmonary infection. Conclusion：The effect of talcum powder in geriatric pneumothorax is definite. By utilizing correctly and reducing adverse reaction as fax as possible, this method is a good for treating geriatric pneumothorax.
