摘  要:目的分析电极阻抗测试和神经反应遥测(NRT)技术在人工耳蜗手术中的应用,评估人工耳蜗植入术术中电极阻抗测试、NRT的临床意义,探讨术中检测的方式及注意事项。方法对12例植入Nucleus24型人工耳蜗的患者于术中及术后1个月进行电极阻抗测试、NRT,并比较术中、术后ECAP阈值的差异。结果91.7%(11/12)的患者术中电极阻抗测试正常;8.3%(1/12)的患者其中1个电极出现开路。91.7%(11/12)的患者记录到良好的NRT反应波形;8.3%(1/12)的患者术中NRT测试未获得分化良好的ECAP。术中、术后ECAP阈值具有显著的相关性。结论电极阻抗测试可以及时了解电极的工作状况。NRT技术可以有效地应用于Nucleus人工耳蜗植入患者的术中检测,同时协助开机时不能进行心理物理测试的儿童完成映射图的调试。Objective To evaluate the clinical significance of intra-operative monitoring assessment including impedance measurement and neural response telemetry (NRT) during cochlear implant surgery and to explore intra-operative protocols. Methods 12 patients implanted with the Nucleus CI24 cochlear implant system including 1 cochlear malformation and 2 LVAS participated in this study. The impedance of electrodes and NRT were measured separately during the surgery and after the surgery. Results Electrode impedance measurements indicated that 8.3%(1/12) patients' electrode arrays had broke while 91.7%(11/12) patients' were normal. Clear waveform of electrically evoked compound action potential (ECAP) could not he recorded for 8.3% (1/12) patient during the NRT test. There is significant correlation on the electrically evoked compound action potential (ECAP) between the patient during and after the surgery. Conclusion Impedance measurement can understand the working conditon of electrode arrays in the first time and NRT make an importment role in intra-operative mornitor.
