多普勒组织成像技术对糖尿病患者左室舒张功能的评价作用(附80例报告)

摘  要:目的探讨多普勒组织成像技术(TDI)对评价糖尿病患者左室舒张功能的临床价值。方法对80例糖尿病患者及50例正常人进行常规超声心电图及多普勒组织成像检查,测量各房室内径、室壁厚度、射血分数、左室收缩期二尖瓣环舒张早期运动速度(Ea)、舒张晚期运动速度(Aa)及Ea/Aa,并与血流多普勒指标E波速度(E)、A波速度(A)、E/A进行比较,两组指标比较采用成组t检验。结果与正常组相比,糖尿病组舒张期E、E/A降低(P<0.05),Ea、Ea/Aa明显降低(P<0.01),Aa无明显差异。结论TDI检测出糖尿病患者早期存在左心室舒张功能异常,有助于舒张功能受损程度的判断;TDI技术在评价左室舒张功能方面较二尖瓣血流频谱更敏感。Objective To explore the clinical value of tissue Doppler imaging (TDI) in assessing the left ventricular diastolic function in patients with diabetes mellitus (DM). Methods The diameters of atrium and ventricle, LVEF and E, A, E/ A, Ea, Aa, Ea/Aa were recorded with the use of ultrasound in 80 patients with diabetes mellitus and 50 normal volunteers. The peak diastolic velocities of mitral annulus were measured by Doppler tissue imaging, respectively. The myocardial velocities were determined with the use of analysis of grouped t test. Results Compared with healthy subjects, the E, E/A in patients with DM decreased (P〈0. 05), the Ea, Ea/Aa in patients with DM decreased significantly (P〈0. 01), peak myocardial velocities of mitral annulus during atrialcontraction (Aa) were similar between normal subjects and patients with DM. Conclusion Tissue Doppler imaging can show the change of left ventricular diastole function in DM and help to evaluate the degree of left ventricular dysfunction. Tissue Doppler imaging is more reliable than pulsed Doppler echocardiography in the assessment of the left ventricular diastolic function.
