摘　　要：目的探讨建立寰枢关节骨错缝的一种可靠的测量方法。方法随机抽取47例门诊颈椎病患者的开口位X线片,用数码相机翻拍后输入计算机,采用Coreldraw12.0图像处理软件进行寰椎、齿突轴线共线法和比值法两种测量方法的分析比较。结果比值法的灵敏度(真阳性率)为96.88%,特异度(真阴性率)为73.33%,符合率为89.36%,假阳性率为26.67%,假阴性率为3.12%,似然比为3.63,正确诊断指数为70.21%。结论寰枢关节骨错缝的比值测量法具有很高的灵敏度和特异度,不仅能客观反映寰枢关节骨错缝的程度,而且也可为评价手法矫正寰枢关节骨错缝的疗效提供客观的量化依据。Objective To seek a quantitative measurement of articulatio atlantoepistmphica semidislocation by imaging. Methods Forty-seven mouth-open X-ray pictures of cervical spondylopathy patients were chosen and digitally computerized to compare the results of axiscollineation measurement and ratio measurement on the platform of Corel draw12.0. Results As to the ratio measurement,the sensitivity was 96.88% ,the specificity 73.33%, the coincidence rate 89.36% ,the false positive rate 26.67% .the false negative rate 3.12%, the likelihood ratio 3.63,and the Youden＇s index was 70.21%. Conclusion The ratio measurement has a better sensitivity and specificity, and can not only objectively reflect the degree of articulatio atlantoepistrophica semidislocation, but also provide an objective reference for assessment of the treatment results of articulatio atlantoepistrophica semidislocation.
