氯氮平所致糖代谢异常患者血清IL-6与HbA1c异常的关系

摘　　要：目的:探讨精神分裂症经氯氮平治疗后血清白细胞介素6(IL-6)变化水平与糖化血红蛋白(HbA1 c)异常在糖代谢异常中的相互关系及作用机制。方法:采用酶联免疫吸附法(ELIAS)测定60例精神分裂症患者氯氮平治疗8周前、后IL-6变化及HbA1 c,同时记录治疗前、后空腹血糖(FPG)、糖耐量试验(2hPG)结果。结果:治疗后IL-6水平显著升高(t=2.958,P<0.005),HbA1 c异常患者糖代谢异常发生率为26%,远较HbA1 c正常者8%的发生率高,差异有统计学意义(χ2=13.5412,P<0.005)。糖代谢异常患者HbA1 c异常与IL-6呈正相关(t=4.6793,r=0.8014)。结论:氯氮平对精神分裂症患者血浆IL-6水平有显著影响,氯氮平所致IL-6升高与HbA1 c异常共同构成精神分裂症患者糖代谢异常的危险因素。Objective： To explore the relationship and the acting - mechanism between the plasma changes of Interleukin - 6（IL - 6） and abnormal glycosylated hemoglobin（HbAlc） in Clozapine induced Abnormal Sugar metabolism of schizophrenia after elozalpine treatment.Motheds： 60 Schizophrenicpatients was determined IL- 6 and HbAlc by Enzyme- linked Immunosorbent Assay before and after the study, as the fasting plasma glucose（FPG） and 2 - hour plasma glucose in the oral glucose tolerance test （2hPG） .Results： After 8 weeks all of the levels of IL - 6 became more higher（ t = 2. 958, P 〈 0.005） ; The rates of abnormal sugar metabolism is 26% in abnormal HbAlc higher than the 8% in normal, it is statistic differenee（X^2 = 13. 5412, P 〈 0. 005）. There was significant consistent between IL - 6 and abnormal HbAlc （ t = 4. 6793, r = 0. 8014）. Conclusion： Clozapine has significant influence on the plasma IL - 6 in Schizophrenia, clozapine induced IL - 6 increase and abnormal HbAlc may be the risk factor of abnormal sugar metabolism in Schizophrenia together.
