摘  要:背景:现有的根充材料对尖周组织有不同程度的不良刺激,不能引导骨的生长及尖周区破坏骨的再生,且使用时操作不易掌握。目的:探讨珊瑚糊剂根管充填后与尖周组织的相容性及引导骨生长的能力。设计:对比观察。单位:武汉大学人民医院口腔科。材料:实验于 2002-09/2006-01 在武汉大学口腔生物医学工程教育部重点实验室完成。采用健康成年草狗 8 条,雌性,体质量 15 kg 左右,由解放军广州军区武汉总医院提供(SYXK(鄂)2003-0007)。以根管为单位数,每条狗 32 个根管,合计 256 个根管。方法:静脉推注戊巴比妥钠(30 mg/kg),行全身麻醉,气管插管,按以下设计进行根充。实验组(n=128)采用复方珊瑚糊剂(珊瑚 40 g:碘仿 8 g,由武汉学口腔生物医学工程教育部重点实验室提供);对照组(n=128)用牙胶尖十氧化锌丁香油糊剂。分 2,4,12,24 周 4期,每期麻醉后处死 1 只,拍 X 射线牙片,并行光镜观察。主要观察指标:①根尖周炎症:按高倍视野炎症细胞计数,分为轻度( 〈 100 个/mm2),中度(100~200 个/mm2),重度( 〉 200 个/mm2)。②根尖周组织破坏和修复再生情况。③复方珊瑚糊剂在根尖部的骨替代情况。结果:8 条草狗均进入结果分析。①X射线片根尖暗影情况:实验组各期未见明显根尖暗影。对照组在 12,24 周各有 1 例根尖周暗影。②组织病理学观察:根充后 2 周:除实验组珊瑚颗粒散在尖周外,各组均有炎症细胞浸润,以中性白细胞为主,但实验组较少,属轻度,对照组为中度。根充后 4 周:实验组珊瑚颗粒周围有少量炎症细胞浸润,对照组有大量炎症细胞聚集于根尖周组织。根充后 12 周:实验组根尖周有骨质沉积并与珊瑚颗粒紧密结合,根尖孔变小,无炎症细胞浸润,对照组其尖端上方仍有炎性细胞包绕。根充后 24 周:实验组根充区未见珊瑚颗粒,有大BACKGROUND: The root canal filling materials available have different degrees of noxious stimulation to periapical tissue and cannot induce growth and regeneration of bone at the periapical region, Moreover, the operation of canal filling is not easily controlled, OBJECTIVE: To investigate biocompatibility and osteoconductivity between periapical tissue and compound coral paste after root canal filling, DESIGN: Controlled observation. SETTING: Department of Stomatology in the Renmin Hospital of Wuhan University. MATERIALS: The experiment was carried out in the Key Laboratory for Oral Biomedical Engineering by the State Ministry of Education, School of Stomatology in Wuhan University from September 2002 to January 2006. Eight adult female mongrel dogs, weighed 15 kg, were offered by the Wuhan General Hospital of Guangzhou Military Area Command of Chinese PLA (SYXK-2003-0007). Each dog contained 32 root canals, and totally 256 root canals were utilized in this study, METHODS: Experimental animals underwent general anesthesia by the intravenous injection of sodium pentobarbital (30 mg/kg), then tracheal intubation was utilized for the root canal filling, and the canals were randomly distributed into two groups: experimental group (n=128) with compound coral paste (coral 40 g and iodoform 8 g, offered by the Key Laboratory for Oral Biomedical Engineering by the State Ministry of Education, School of Stomatology in Wuhan University); control group (n=128) with gutta-percha point+zinc oxide-eugenol sealer. One animal was sacrificed by anesthetic overdose at 2, 4, 12 and 24 weeks after root canal filling, dental film by X-ray and light microscope were used for the examination. MAIN OUTCOME MEASURES: ①Periapical inflammation: Cells were counted in a high power field, and three levels were defined as mild (〈 100 cells/mm^2), moderate (100-200 cells/mm^2) and severe (〉200 cells/mm^2).②destruction and regeneration of periapical tissues.③bone substitute of compoun
